About the service

Rangeford Care Wadswick Green is a domiciliary care agency providing personal care to older people living in their own homes within an assisted living development. Not everyone who used the service received personal care. CQC only inspects where people receive personal care. This is help with tasks related to personal hygiene and eating. Where they do we also consider any wider social care provided. At the time of the inspection 16 people were receiving personal care.

People’s experience of using this service and what we found

People were supported by staff who had the skills and knowledge to meet their needs safely and effectively and followed best practice. Views of people were valued and used to make improvements to their care and support.

People's care and support needs were being met in line with their personal preferences. The service responded promptly when people’s needs changed. The service worked well with other health professionals to make sure people’s care needs were met.

People were actively involved in making decisions about their care. They were supported to have maximum choice and control of their lives and staff supported them in the least restrictive way possible and in their best interests; the policies and systems in the service supported this practice.

Staff members felt they were valued and respected by the management team, who sought their involvement to improve and develop the service.

People were supported by staff who were caring, kind, respected their dignity and privacy, and promoted their independence.

The service was well managed, by a registered manager and management team who were described as ‘supportive’, ‘approachable’ and, ‘brilliant’. Quality assurance systems, including audits, feedback from people who used the service and staff were all used to make continuous improvements to the quality of the service people received.

Rating at last inspection: The last inspection report was published in June 2017 and the service was rated Good.

Why we inspected: This was a planned inspection. The service was rated Good overall.

Follow up: We will continue to monitor intelligence we receive about the service until we return to visit as per our re-inspection programme. If any concerning information is received, we may inspect sooner.

During a routine inspection

Rangeford Care is registered to provide personal care to people living in their own homes. The services provided included support with personal care such as assistance with bathing and dressing and support with meal preparation, administering medicine and accessing healthcare appointments. The offices were located in Wandswick Green, which is a purpose built village for people over the age of 55. At the time of our inspection there were thirteen people receiving a service from the agency.

This inspection took place on 16 May 2017 and was announced. The provider was given short notice because the location provides domiciliary care services. We wanted to make sure the registered manager would be available to support our inspection, or someone who could act on their behalf. This also gave us the opportunity to ensure people and staff would be available to talk with us. This was the first inspection since the location had been registered as a domiciliary care provider in April 2016.

A registered manager was employed by the service who was present throughout the inspection. A registered manager is a person who has registered with the Care Quality Commission to manage the service. Like registered providers, they are ‘registered persons’. Registered persons have legal responsibility for meeting the requirements in the Health and Social Care Act 2008 and associated Regulations about how the service is run.

People received care and support from staff who had got to know them well. People’s dignity, privacy and independence were promoted and people were treated with respect.

Staff had the knowledge and confidence to identify safeguarding concerns and acted on these to keep people safe. Risks to people’s personal safety had been assessed and plans were in place to minimise these.

Medicines were managed safely and administered by trained staff. People received their medicines as prescribed and in their preferred manner. People’s health care needs were monitored by staff during their visits. Any concerns were raised with the registered manager, appropriate health professionals and where appropriate the person’s relative.

People received their care at the correct time. There were enough staff deployed to fully meet people’s health and social care needs.

Care and support plans were personalised and detailed daily routines specific to each person. Handover between staff at the start of each shift ensured important information was shared and acted upon where necessary. People were involved in planning and reviewing what support they needed.

Safe recruitment procedures were followed and staff had the relevant checks in place prior to them commencing employment. Staff had received, or had planned, supervision (one to one meetings) and appraisals with their line manager. People were supported by staff who had access to a range of training and had received direct observations of their working practices to ensure they were skilled and competent.

The service was working within the principles of the Mental Capacity Act 2005. Staff had an understanding of the Mental Capacity Act 2005 and explained how they supported people to make decisions regarding their daily living and support.

There were processes in place to monitor the quality and safety of the service to ensure any shortfalls were identified and acted upon. People and those important to them had opportunities to feedback their views about the service. People’s concerns and complaints were investigated and responded to in good time.

During a routine inspection

The inspection took place on the 15 and 16 March 2016 and was announced. This was because the location provides domiciliary care services. We wanted to make sure the manager would be available to support our inspection, or someone who could act on their behalf. This was the first inspection since the location had been registered as a domiciliary care provider in April 2015.

Rangeford Care is registered to provide personal care to people in their own homes. The agency also provides other support which includes supporting people with preparing their meals, administering medicines and support to access healthcare appointments. The agency office is located in Wadswick Green, which is a purpose built village for people over the age of 55. Their aim is to provide bespoke care and support services tailored around the needs and wishes of the individuals living at Wadswick Green. At the time of our inspection there were nine people receiving a service from the agency.

People all spoke positively regarding the care and support they received. They said they received the support required from staff, which enabled them to remain independent and living in their own homes. People described staff as “Caring, thoughtful and friendly” and that nothing was “too much trouble”.

People told us they felt safe whilst they were receiving a service from staff working for Rangeford Care. People were protected from the risk of harm or abuse because systems were in place to manage any safeguarding concerns. Staff were aware of their responsibility to report any concerns they had about people’s safety and welfare.

People were cared for and supported by staff who were kind and caring. They respected people’s privacy and dignity. Person centred care plans were in place to instruct staff on how best to support people and meet their needs. Staff confirmed they had access to care plans and read them regularly to keep themselves up to date with people’s care and support needs.

Staff confirmed they received a comprehensive induction, training and regular supervision and we saw records to support this. They were happy with the training which they felt supported them to carry out their role correctly. Some staff were accessing a national qualification in health and social care.

There were enough staff deployed to fully meet people’s health and social care needs. The registered manager and provider had systems in place to ensure safe recruitment practices were followed.

Medicines were managed safely and administered by trained staff. People received their medicines as prescribed and in their preferred manner. They were supported to be independent and manage their own medicines were appropriate. People were supported to access health care services and maintain good health as required.

The registered manager had systems in place to monitor the quality of service provided. There were opportunities for people who used the service and staff to express their views about the service provided by the agency. People told us they were confident that if they expressed concerns or complaints they would be dealt with appropriately by staff and the registered manager.
