Responsive
People had access to paper versions of their care records, not everyone used the online version. Systems were being introduced to provide a timely review of new care plans to identify any changes required to meet people's needs. The provider planned get together in the lo community for people, their families and staff to attend.

Equity in experiences and outcomes
People told us they were asked by the provider if they needed extra help. Where people had raised concerns these were acted on by the provider.
People were invited to attend a 'get together' organised by the provider. One person told us they had attended an organised event, supported by their family member.
The provider implemented a system which facilitated ongoing monitoring of people's care need delivery by staff. The provider was developing a model to utilise staff time to provide community access and companionship calls to improve outcomes for people.
The provider worked with specialists to provide specialist support as required, this included the development of training for staff to be able to support people safely.
The provider was developing a system of an audit for all new care plans. The aim was to complete a review within the first month of a new care plan being set up. This was intended to identify and trigger actions in response to changes in peoples needs and an increased understanding of clients preferences.
