Involving people to manage risks
The judgement for Involving people to manage risks is based on the latest evidence we assessed for the Safe key question.
Safe environments
At the inspection on 23 September 2026, we found the following:
The practice had improved the systems to identify and manage risks. In particular, risks associated with fire safety, maintenance of equipment and premises, medical emergencies, hazardous substances and staff recruitment.
Hazardous substances were clearly labelled and stored safely. Improvements had been made to ensure all hazardous substances had been risk assessed and a safety data sheet available.
Staff could access emergency equipment and medicines that were checked in line with national guidance. They knew how to respond to a medical emergency, and staff had completed training in emergency resuscitation and basic life support every year.
The management of fire safety had improved. The practice was carrying out the relevant in-house testing of the fire detection system. A fire risk assessment had been conducted in March 2026 and some of the recommendations had been acted upon including the installation of a new fire alarm and emergency lighting system, along with some new fire extinguishers. An electrical installation condition report (EICR) for the premises had also been conducted in March 2026. The provider should ensure they continue to work through the recommendations from the fire risk assessment and EICR in a timely manner.
Improvements had been made to ensure the compressors had undergone their service and pressure vessel inspection.
Safe and effective staffing
At the inspection on 23 September 2026, we found the following:
The practice had a recruitment policy and procedures that reflected relevant legislation, to help them employ suitable staff. We were unable to check if these were now consistently followed due to no new recruitment since the previous inspection. We noted some improvements had been made to ensure most clinical staff had appropriate immunity to vaccine-preventable blood-borne viruses. However, we noted immunity levels to Hepatitis B had still not been procured for 1 staff member. The practice acted immediately and sent evidence of a Hepatitis B risk assessment whilst they sourced immunity levels.
Improvements had been made to the oversight of staff training and continuing professional development.
Infection prevention and control
At the inspection on 23 September 2026, we found the following:
Improvements had been made to the procedures to reduce the risk of Legionella, or other bacteria, developing in water systems. A Legionella risk assessment had been conducted in April 2026 to cover all floors of the practice, and the recommendations had been acted upon. Infrequently used outlets were now being flushed.
We noted equipment was consistently maintained and serviced in line with manufacturers’ instructions. In particular, the autoclaves had undergone their service, pressure vessel inspection and validation.
Medicines optimisation
The judgement for Medicines optimisation is based on the latest evidence we assessed for the Safe key question.
