Effective – this means we looked for evidence that people’s care, treatment and support achieved good outcomes and promoted a good quality of life, based on best available evidence. This is the first assessment for this newly registered service. This key question has been rated good. This meant people’s outcomes were consistently good, and people’s feedback confirmed this.

Assessing needs

The provider made sure people’s care and treatment was effective by assessing and reviewing their health, care, wellbeing and communication needs with them. People’s needs were assessed before they moved into the home and were reviewed regularly, and as their needs changed. The new provider was in the process ensuring all care plans were reviewed and in line with their approach and expectations.

Delivering evidence-based care and treatment

The provider planned and delivered people’s care and treatment with them, including what was important and mattered to them. They did this in line with legislation and current evidence-based good practice and standards. People’s dietary needs, including cultural needs, and preferences were assessed and planned for. We observed people had a positive mealtime experience. Staff created a calm and social atmosphere. Staff supported people in a sensitive, unhurried way. The lunch time meal appeared appetising and well presented. Where people had specific dietary needs, these were catered for in a creative way. For example, pureed meals were available in a style which matched the meal so people with modified diets were able to see the meal as a plate of food rather than just puree.

How staff, teams and services work together

The provider worked well across teams and services to support people. They made sure people only needed to tell their story once by sharing their assessment of needs when people moved between different services. There was clear communication between staff and the electronic care planning system enabled information to be shared effectively. Handover meetings between staff teams provided an opportunity for regular updates and discussions about any changes in people’s needs.

Supporting people to live healthier lives

The provider supported people to manage their health and wellbeing to maximise their independence, choice and control. Staff supported people to live healthier lives and where possible, reduce their future needs for care and support. People were engaged in activities which supported them to improve their health and well-being. Activities enabled people to participate at their level. This enabled people with physical, sensory and cognitive impairments to be involved and enjoy the sessions. We observed people enjoying a musical activity where they sang, danced and were drinking mocktails and smoothies.

Monitoring and improving outcomes

The provider routinely monitored people’s care and treatment to continuously improve it. They ensured outcomes were positive and consistent, and that they met both clinical expectations and the expectations of people themselves. Care plans and daily notes included information and updates about people’s on-going care needs. Referrals were made to local health and care services when people needed specific clinical support or assessments.

The provider told people about their rights around consent and respected these when delivering person-centred care and treatment. People were asked to confirm their consent to receive care and support from the provider. Where people lacked capacity best interest decisions were recorded, which included input from people’s families and advocates.
