Assessing needs

The provider made sure people’s care and treatment was effective by assessing and reviewing their health, care, wellbeing and communication needs with them.

People’s needs were assessed by the registered provider prior to them starting with the service. The assessment covered all aspects of care and support needs to help ensure appropriate support could be provided. The registered provider explained that care records would be reviewed and updated as people’s needs changed.

Delivering evidence-based care and treatment

The provider did not always plan and deliver people’s care and treatment with them, including what was important and mattered to them.

The registered provider demonstrated a limited awareness of evidence-based best practice and frameworks, such as NICE guidance. We were not assured care and support provided would consistently be informed by current evidence-based guidance and best practice.

People and, where appropriate, their relatives were involved in the assessment process to help ensure care reflected individual needs and preferences.

How staff, teams and services work together

The registered provider described how they would liaise with external professionals. However, these arrangements had not yet been implemented or tested, and there were no formal processes in place. Therefore, there was insufficient evidence available at the time of this assessment to enable a judgement or provide a rating for this quality statement.

Supporting people to live healthier lives

The provider supported people to manage their health and wellbeing to maximise their independence, choice and control. Staff supported people to live healthier lives and where possible, reduce their future needs for care and support.

People’s care plans included detailed information about their specific health conditions and provided guidance on how they liked to be supported. Daily care notes recorded how people were supported to live a healthier life.

Monitoring and improving outcomes

The provider did not always routinely monitor people’s care and treatment to continuously improve it. They did not always ensure that outcomes were positive and

consistent, or that they met both clinical expectations and the expectations of people themselves.

The registered provider described how they would monitor and improve outcomes for people; however, their knowledge in this area was limited and there were no formal processes in place to support this. As the registered provider had only recently started delivering regulated activity, these arrangements had not yet been implemented or tested.

The provider did not always tell people about their rights around consent and did not always respect their rights when delivering care and treatment.

The registered provider demonstrated a limited understanding of the Mental Capacity Act (MCA), and records reviewed showed that documentation had not been completed correctly. In addition, the governance systems in place had not identified these issues, which meant there was a lack of effective oversight to ensure compliance with legal requirements.

Care records evidenced consent was gained and was respected when delivering care.
