Effective – this means we looked for evidence that people’s care, treatment and support achieved good outcomes and promoted a good quality of life, based on best available evidence.

At our last assessment we rated this key question inadequate. At this assessment the rating has changed to requires improvement. This meant the effectiveness of people’s care, treatment and support had improved but did not always achieve good outcomes or was inconsistent.

Assessing needs

The provider made sure people’s care and treatment was effective by assessing and reviewing their health, care, wellbeing and communication needs with them. We found evidence of professional’s visits within people’s notes showing involvement from multiple different professionals such as Advanced Nurse Practitioners, District Nurses, General Practitioners, chiropodist, social workers, dieticians and Deprivation of liberty assessors. Care plans had improved since our last inspection containing good detail and were person centred. Care plans detailed people’s preferences and people’s voice. We saw evidence of people and their relatives being included in discussions about their care. One person told us the manager often came to [Name’s] room to ask them how things are and if there is anything that they would like to change. The person said, “I asked for my bed to be moved in my room, so I had a better view of outside. It was all sorted out within 10 minutes; I am so happy.”

Since our last inspection the management of incident and accidents had improved. Lessons were learnt and audits carried out however, further embedding was required to ensure good practice. Staff were knowledgeable of how to keep people safe and knew what to do if they had a safeguarding concern. Staff said they would report to their senior, manager or go higher if nothing had been done.

How staff, teams and services work together

The provider worked well across teams and services to support people. Staff meetings were held on a regular basis. Staff told us the information shared during the handovers and huddles between shifts enabled them to meet people’s needs and respond to any changes in their health or wellbeing. Staff told us, “We work as a team. The management team always support us, whenever we need them on the floor they help us. They guide us on what to do if we aren’t sure of something” and “We read the care plans to understand the person’s needs or ask the senior.”

Monitoring and improving outcomes

People were involved in monitoring and reviewing their own health and wellbeing needs as possible. The service worked with healthcare professionals to ensure reasonable adjustments were made to enable people to access healthcare. The service made referrals to appropriate health services when people’s needs changed. For example, 1 person had been having regular falls and a referral to the falls team had been completed to gain further advice on how best to support the person.
