Effective – this means we looked for evidence that people’s care, treatment, and support achieved good outcomes and promoted a good quality of life, based on best available evidence.

This is the first assessment for this newly registered service. This key question has been rated as good, this meant people’s outcomes were good, and people’s feedback confirmed this.

Assessing needs

The provider made sure people’s care and treatment was effective by assessing and reviewing their health, care, wellbeing, and communication needs with them.Information gathered during assessment was used to create care plans that guided staff on how to care for people. People told us that they were involved in discussions about their care and support needs.

Delivering evidence-based care and treatment

Staff delivered care in line with peoples care plans. Care records contained information about people health conditions and the support they required, this helped staff provide care that reflected their needs.Staff had completed a range of training relevant to their roles. This was both mandatory and practical training to support them in providing safe care. Staff told us that the training helped them in their role and to understand people’s needs.

How staff, teams and services work together

Staff and management communicated with each other to ensure peoples care needs were understood and met. Staff told us they received good support from management and were able to contact them for advice or support when they needed it.People told us that other visiting professionals were happy with the support they were receiving from the service.

Supporting people to live healthier lives

Staff monitored people’s wellbeing during care visits. Records demonstrated that care was delivered in line with care plans. The manager told us that they worked closely with relatives and kept regular contact with them to monitor peoples’ conditions, this meant they maintained oversight of the service. Staff told us that they would contact the office for guidance if they discovered anything that needed to be reported to a nurse but would dial 999 in an emergency.

People told us that they were always encouraged to keep doing what they could manage, meaning they would not lose that ability, and they were supported enough to be safe and stay independent at home.

Monitoring and improving outcomes

The provider monitored people’s care and treatment to continuously improve it, though some areas required improvements.Care plans reflected details of how the person wanted to be supported, people told us that the support provided to them was comforting and even small gestures improved their outcomes.

The provider told people about their rights around consent and respected these when delivering person-centred care and treatment.There were no people that required best interest decisions or mental capacity assessments, however staff confirmed they had received training about the requirements of the Mental Capacity Act 2005 (MCA) and we saw completion certificates.People told us that they were always asked for consent before any task was completed by staff
