Learning culture

Score: 3

The provider had a proactive and positive culture of safety, based on openness and honesty. Staff listened to concerns about safety and investigated and reported safety events. Lessons were learnt to continually identify and embed good practice. Incidents and safeguarding concerns were logged and investigated, with action taken to address concerns. Learning was identified and shared to help improve people’s care, including reinforcing expected working practice and professionalism.

Safe systems, pathways and transitions

The provider worked with people and healthcare partners to establish and maintain safe systems of care, in which safety was managed or monitored. They made sure there was continuity of care, including when people moved between different services. People’s needs were assessed prior to the service starting, which included input from health and social care professionals. This helped ensure the provider was able to meet their needs appropriately.

Safeguarding

The provider worked with people and healthcare partners to understand what being safe meant to them and the best way to achieve that. Staff concentrated on improving people’s lives while protecting their right to live in safety, free from bullying, harassment, abuse, discrimination, avoidable harm and neglect. The provider shared concerns quickly and appropriately. Safeguarding concerns were shared and managed appropriately. The service had a safeguarding policy and staff had completed safeguarding training. Previous safeguarding concerns had been investigated and resolved. People and relatives felt the service was safe. A relative said, “(Their family member felt) totally safe at all times."

Involving people to manage risks

The provider worked with people to understand and manage risks by thinking holistically. Staff provided care to meet people’s needs that was safe, supportive and enabled people to do the things that mattered to them. Potential risks were identified and assessed, including falls, mobility, nutrition, skin integrity. Risk assessments included details of the measures required to keep people safe.

Safe environments

The provider detected and controlled potential risks in the care environment. They made sure equipment, facilities and technology supported the delivery of safe care. Environmental risk assessments were completed when the service started, which included guidance for staff on safe access and lone-working arrangements.

Safe and effective staffing

The provider made sure there were enough qualified, skilled and experienced staff, who received effective support, supervision and development. They worked together well to provide safe care that met people’s individual needs. People and relatives said staff were reliable, punctual and stayed for the full call. A person said, "They are mostly on time and usually stay over the time allocated rather than under." Staff had completed training appropriate to their role and had regular 1:1 supervision sessions.

Infection prevention and control

The provider assessed and managed the risk of infection. They detected and controlled the risk of it spreading and shared concerns with appropriate agencies promptly. The provider had infection prevention and control (IPC) policies and staff had completed IPC training.

Medicines optimisation

Score: 3

The provider made sure that medicines and treatments were safe and met people’s needs, capacities and preferences. Staff involved people in planning, including when changes happened. Medicines were administered safely. People received their medicines when they were due from trained and competent staff. Management completed checks to ensure staff administered medicines safely.
