During a routine inspection

We carried out this announced comprehensive inspection on 2 June 2023 under section 60 of the Health and Social Care Act 2008 as part of our regulatory functions.

We planned the inspection to check whether the registered practice was meeting the legal requirements in the Health and Social Care Act 2008 and associated regulations.

The inspection was led by a Care Quality Commission (CQC) inspector who was supported by a specialist dental advisor.

To get to the heart of patients’ experiences of care and treatment, we always ask the following 5 questions:

  • Is it safe?
  • Is it effective?
  • Is it caring?
  • Is it responsive to people’s needs?
  • Is it well-led?

These questions form the framework for the areas we look at during the inspection.

Our findings were:

  • The dental clinic appeared clean and well-maintained.
  • The practice had infection control procedures which reflected published guidance.
  • Staff knew how to deal with medical emergencies. Appropriate medicines and life-saving equipment were available. Staff were not recording the fridge temperature where the glucagon was stored and the bodily fluids and mercury spillage kits were out of date.
  • The practice had systems to manage risks for patients, staff, equipment and the premises.
  • Safeguarding processes were in place and staff knew their responsibilities for safeguarding vulnerable adults and children.
  • The practice had staff recruitment procedures which reflected current legislation.
  • Clinical staff provided patients’ care and treatment in line with current guidelines. However, not all dentists kept detailed patient care records in line with recognised guidance.
  • Patients were treated with dignity and respect. Staff took care to protect patients’ privacy and personal information.
  • Staff provided preventive care and supported patients to ensure better oral health.
  • The appointment system worked efficiently to respond to patients’ needs.
  • The frequency of appointments was agreed between the dentist and the patient, giving due regard to National Institute of Health and Care Excellence (NICE) guidelines.
  • There was effective leadership and a culture of continuous improvement.
  • Staff felt involved, supported and worked as a team.
  • Staff and patients were asked for feedback about the services provided.
  • Complaints were dealt with positively and efficiently.
  • The practice had information governance arrangements.

Background

Cosmo-Dent Dental Practice is in Halesowen, West Midlands and provides NHS dental care and treatment for adults and children.

There are steps leading up to the entrance, therefore the practice is not accessible for people who use wheelchairs and those with pushchairs. The practice has made reasonable adjustments to support patients with other access requirements such as a hearing loop.

The dental team includes 3 dentists, 3 dental nurses, 1 practice manager and 1 receptionist. The practice has 2 treatment rooms.

During the inspection we spoke with 2 dentists and 3 dental nurses. We looked at practice policies, procedures and other records to assess how the service is managed.

The practice is open:

Monday to Friday from 9am to 5.30pm

There were areas where the provider could make improvements. They should:

  • Implement an effective system for monitoring and recording the fridge temperature to ensure that medicines and dental care products are being stored in line with the manufacturer’s guidance.
  • Improve the practice protocols regarding auditing patient dental care records to check that necessary information is recorded.
  • Improve the security of NHS prescription pads in the practice and ensure there are systems in place to track and monitor their use.

During a routine inspection

This dental practice location is a small dental practice, owned and operated by one person. This dental practice is situated on the main high street in Oldbury. The practice is made up of a small team which includes two dentists, three nurses and a manager.

This practice provides a full range of general dental assessments and treatments.

The practice location has two treatment rooms. Treatment rooms and the waiting room are all on the first floor so would not be suitable for people who use a wheelchair.

People receiving care and treatment from this practice location pay independently or can access their dentistry through NHS costs. The practice has a current contract with the NHS to provide NHS dental care to people.

As part of our review we spoke with two people by phone (these people also told us about the experiences of other family members) and three people in the waiting room. People spoke highly of this dentist practice. People were satisfied with the care and treatment they had received. Below are a few comments people made;

'I have been using this dentist for about ten years. I like the people they are so friendly and kind. They treat you as a person rather than a number'.

'I really have to travel far to get to my appointments but it is worth the travelling. I have confidence in this dentist and they put me at ease'.

'I travel a way to get to this dentist but I do not want to go anywhere else. They are great people, very friendly and kind. They explain things to you and make you feel comfortable'.

'They are very thorough and highly effective I can not fault them'.

'I am very happy with this dentist and so are other family members. My family recommended this dentist to me and I recommend the dentist to other people'.

'I have been coming to this dentist since I was young. They are great'.

We spoke with a number of staff. They made positive comments about working at this dental practice. They told us that they had worked at the practice for a number of years and really enjoyed working there and their job.

Evidence that we gathered during this review confirmed that people are assessed to decide what dental and oral healthcare needs they may have. Treatments that people may need are discussed with them and are carried out in a satisfactory way.

Overall we found adequate practices regarding infection control and cleaning procedures which demonstrated compliance in this area.
