We found staff had access to safe systems and pathways. There was an open and honest learning culture. Women were appropriately assessed and supported to manage risk. There was a sufficient number of skilled staff to keep women safe. We observed the clinic was clean and medicines were prescribed, stored and administered in line with national guidelines and best practice.

Although the service’s rate of repeat terminations was below the national average, it nonetheless represented a significant proportion of the women accessing the service. We found there was no formal policy or process for supporting these women. Whilst staff reviewed each case to ensure women were safeguarded, there was no standardisation of care. The uptake of Long-acting reversible contraception had significantly decreased, however, since the assessment, managers implemented action plans to improve these numbers.
