Involving people to manage risks

The provider worked with people to understand and manage risks by thinking holistically. Staff provided care to meet people’s needs that was safe, supportive and enabled people to do the things that mattered to them.

At our last assessment, the provider did not ensure all risks to people’s safety and wellbeing were identified or provided enough detail as to how these should be mitigated. For example, where people could become anxious, distressed and exhibited behaviours that could place themselves and others at risk of harm.

During this assessment, we found risks to people's safety were assessed to help ensure they remained as safe as possible. Risks associated with individuals' care and support were reviewed following any incidents or accidents, in line with the organisation's procedures.

Appropriate measures were in place to enable staff to provide safe and effective support. Risk assessments covered key areas such as peoples’ health conditions, personal care, completing household chores and medicines management. These assessments provided staff with clear guidance on the actions required to minimise risks and promote people's safety and wellbeing.

Safe environments

The provider detected and controlled potential risks in the care environment. They made sure equipment, facilities and technology supported the delivery of safe care.

At our last assessment, the emergency exit fire door in the utility room was jammed and could not be opened and the light to the top floor was not working.

During this assessment, we found the provider had taken action and installed new sensor lights which activates automatically when it detects movement, and a new fire exit door had also been installed.

Refurbishment work of the service was ongoing. For example, the flooring in people’s bedrooms had been replaced.

Safe and effective staffing

The provider made sure there were enough qualified, skilled and experienced staff, who received effective support, supervision and development. They worked together well to provide safe care that met people’s individual needs.

At our last assessment, the provider did not always make sure there were enough qualified, skilled and experienced staff.

During this assessment, we found there were enough staff employed by the service to ensure the needs of people were met and they were supported safely. People were supported by the same staff team to help give continuity of care. The provider had reviewed the staffing level and break times to ensure people’s needs were met safely including people who were assessed as requiring one-to-one support.

People were supported by staff that had received appropriate training and support to do their jobs and meet people’s needs. Staff received regular one to one supervision to discuss their role and development needs.

Infection prevention and control

The provider had an effective approach to assessing and managing the risk of infection, which is in line with current relevant national guidance.

At our last assessment, the provider did not have an effective approach to assessing and managing the risk of infection as we identified some concerns during our visits. We found unlabelled and uncovered food in the fridge, and this posed a significant breach of food safety and hygiene regulations, presenting high risks to people who used the service. We also found food temperatures were not always being monitored. There were missing entries.

During this assessment, we found food were being stored in line with the food hygiene regulations. Food temperatures were being monitored more closely by the registered manager.

Medicines optimisation

The provider made sure that medicines and treatments were safe and met people’s needs, capacities and preferences.

At our last assessment, we found systems were not robust enough to demonstrate medicines were managed safely and effectively. This placed people at risk of harm. We found expired medicines and ‘when required’ basis (PRN) protocols were not detailed and did not inform on the actions staff needed to take if the medicine did not work.

During this assessment, we found there were suitable arrangements for the safe storage, management and disposal of medicines. People were supported to take their medicines by staff who were trained to do so safely.

Any medicines prescribed to be given as necessary were monitored and guidance explained when these medicines should be given. We saw the registered manager carried out regular audit of medicines, including people’s medicines administration records. Medicines were stored appropriately in a lockable metal cupboard.

Records of medicine administration and stock were kept, to show medicines were administered in accordance with people’s prescriptions and available when people needed them.
