Safe – this means we looked for evidence that people were protected from abuse and avoidable harm.

This is the first assessment for this newly registered service. This key question has been rated good. This meant people were safe and protected from avoidable harm.

Learning culture

Score: 3

The provider had a proactive and positive culture of safety, based on openness and honesty. Staff listened to concerns about safety and investigated and reported safety events. Lessons were learnt to continually identify and embed good practice.

Part of the providers quality assurance included lessons learnt so changes could be made to improve practice and support. This included responding to staff concerns by changing staff uniforms to make them more appropriate for the extreme hot weather.

Safe systems, pathways and transitions

The provider worked with people and healthcare partners to establish and maintain safe systems of care, in which safety was managed or monitored. They made sure there was continuity of care, including when people moved between different services.

The provider worked with other professionals to assess people’s needs and ensure smooth transitions for people. This included people leaving hospital to receive end of life care at home.

Safeguarding

The provider worked with people and healthcare partners to understand what being safe meant to them and the best way to achieve that. Staff concentrated on improving people’s lives while protecting their right to live in safety, free from bullying, harassment, abuse, discrimination, avoidable harm and neglect. The provider shared concerns quickly and appropriately.

People felt comfortable and safe with the staff who supported them. One person told us, “I feel safe. It’s their personalities (staff), how they talk to me and treat me that makes me feel safe.”

Staff all received training in recognising and reporting abuse. All said they were confident that any issues reported to the provider would be dealt with to ensure people were protected.

Involving people to manage risks

The provider worked with people to understand and manage risks by thinking holistically. Staff provided care to meet people’s needs that was safe, supportive and enabled people to do the things that mattered to them.

Risk assessments were completed to make sure people could safely receive care in accordance with their wishes. Risk assessments were personalised to each individual and their circumstances.

Safe environments

The provider was fully aware of all potential risks in the care environment and controlled them well. They made sure equipment, facilities and technology supported the delivery of safe care.

The provider carried out risk assessments to make sure staff were working in safe environments. Where people required specialist equipment the staff ensured assessments had been carried out by appropriate professionals.

Safe and effective staffing

The provider made sure there were enough qualified, skilled and experienced staff, who received effective support, supervision and development. They worked together well to provide safe care that met people’s individual needs.

People were supported by staff who had been safely recruited and received training appropriate to their role. People told us they thought staff were competent. One family member said, “The girls know how to use the equipment. They are trained without a doubt.”

There were enough staff to make sure everyone received their care and support within their allocated timeframe. One person commented, “Time keeping, pretty good. Continuity of carers which has helped build up a rapport with them.”

There had been no missed visits.

Infection prevention and control

The provider assessed and managed the risk of infection. They detected and controlled the risk of it spreading and shared concerns with appropriate agencies promptly.

Risks of infection to people were minimised because all staff had received training in infection prevention and control and knew how to work safely.

Staff were supplied with appropriate personal protective equipment (PPE). People said staff wore PPE when they supported them with personal care.

 

Medicines optimisation

The provider made sure that medicines and treatments were safe and met people’s needs, capacities and preferences. Staff involved people in planning, including when changes happened.

People who needed help with medicines were supported by staff who had received appropriate training and had their competency assessed to make sure their practice was safe.

Electronic records were completed by staff when medicines were administered or refused.

Staff worked with other healthcare professionals to make sure people being cared for at the end of their lives received medicines to relieve any pain or discomfort.
