The Registered Provider must not carry on any of the regulated activities they are registered for at Lanchester Medical Centre, Durham Road, Lanchester, Durham, DH7 0LS. This condition will remain in place up to and including Friday 14 November 2025.
The Registered Provider must within 1 month from the date of the imposition of condition 1, send CQC a written action plan that sets out how they will assure CQC they can carry out regulated activities in accordance with the Health and Social Care Act 2008 (Regulated Activities) Regulations 2014 at Lanchester Medical Centre, Durham Road, Lanchester, Durham, DH7 0LS. This must include as a minimum:
a. Long-term conditions management – how you, the provider, will ensure patients with one or more long-term condition(s) will be reviewed and their care and treatment will be aligned to the latest guidance on an ongoing basis. This must include patients living with asthma, diabetes or chronic kidney disease as a minimum.
b. Test results - the system you, the provider, will put in place to ensure that all test results received are reviewed by a suitably skilled and experienced clinician in a timely manner. You must also tell us how you will ensure this system is effective.
c. Medication reviews – the system you, the provider, will put in place to ensure that any medication reviews carried out are completed by a suitably skilled and experienced person. You must also tell us how you will ensure medication reviews are effective.
d. Patient Safety Alerts, including Medicines and Healthcare products Regulatory Agency (MHRA) alerts – the system you, the provider, will put in place to ensure that new and legacy alerts are identified and acted upon on an ongoing basis. You must also tell us how you will ensure this system is effective.
e. NICE guidelines – the system you, the provider, will put in place to ensure all registered patients are treated in line with NICE guidelines. You must also tell us how you will ensure this system is effective.
