During an assessment under our new approach

Face Perfect Clinic is an independent nurse-led clinic located in Leeds, West Yorkshire. The provider is registered with the Care Quality Commission (CQC) under the Health and Social Care Act 2008 in respect of some, but not all, of the services it provides. There are some exemptions from regulation by CQC which relate to particular types of regulated activities and services, and these are set out in Schedule 2 of The Health and Social Care Act 2008 (Regulated Activities) Regulations 2014.
The provider is registered with CQC to deliver the regulated activity of Treatment of Disease, Disorder or Injury. We did not assess, or report on, those services that are outside the scope of CQC registration. Therefore, we only assessed the provision of treatments relating to treatment for hyperhidrosis (excessive sweating), bruxism (excessive teeth grinding or jaw clenching), acne, hormone replacement, and medical weight management.
This is the first assessment for this location since their registration with CQC. At this assessment we reviewed the location’s performance in the key questions safe, effective, caring, responsive and well-led. The service is rated inadequate overall, and in the key questions safe, effective and well-led. The service is rated good in the key questions caring and responsive. We found breaches of regulation in relation to good governance.This service is being placed in special measures. The purpose of special measures is to ensure that services providing inadequate care make significant improvements. Special measures provide a framework within which we use our enforcement powers in response to inadequate care and provide a timeframe within which providers must improve the quality of the care they provide.
The provider did not have effective systems for identifying, acting on and learning from incidents. Policies were not always clear and lacked sufficient information for prescribers to follow safely.
Due to a lack of accurate, complete and contemporaneous clinical records there was no evidence that patients were always informed about risks and how to keep themselves safe, and there was no evidence in clinical records of a prescription being issued. Due to the issues identified during the clinical records review, it was not clear whether the staff delivering some services had the necessary training, knowledge, and experience to do so safely and effectively.
There were appropriate infection prevention and control measures in place.
Clinical record keeping was not adequate which meant that the provider could not demonstrate that they had effectively reviewed patients’ health, care, and wellbeing needs with them. There was no evidence that staff discussed individual risks, complications or side effects with patients, or that they supported them to live healthier lives. It was not clear from patient’s records how the service was monitoring treatments that had been prescribed by them.
The service treated patients with kindness, empathy and compassion and respected their privacy and dignity. Staff told us that the provider considered their wellbeing and that they were well supported in their work.
The provider understood the importance of providing an inclusive approach to care and made adjustments to support patients where required. The provider regularly sought feedback from patients. There was an effective complaints process in place. However, a lack of appropriate record keeping meant that it was not clear whether patients were provided with adequate information about their care and treatment. The provider did not work with other teams and services which supported patients using the service.
The provider was unable to demonstrate that it had clear and effective governance, management, and accountability arrangements in place. There was no collaboration or sharing of information with relevant external stakeholders, and no effective processes in place to support learning, improvement and innovation.
The provider had started to take action following our feedback and concerns from this assessment.
Following our assessment the provider has formally notified CQC that they are no longer providing the hormone replacement and medical weight management services.
