Safe – this means we looked for evidence that people were protected from abuse and avoidable harm. This is the first assessment for this newly registered service. This key question has been rated good. This meant people were safe and protected from avoidable harm.

Learning culture

Score: 3

The provider had a proactive and positive culture of safety, based on openness and honesty. Staff listened to concerns about safety and investigated and reported safety events. Lessons were learnt to continually identify and embed good practice.

 

The provider facilitated opportunities for learning with the staff team. For example, discussing case scenarios and policies in team meetings to evidence understanding. Staff told us there was a positive learning culture.

Safe systems, pathways and transitions

The provider worked with people and healthcare partners to establish and maintain safe systems of care, in which safety was managed or monitored. They made sure there was continuity of care, including when people moved between different services.

 

The provider had appropriate systems in place to facilitate safe systems and pathways of care. For example, working with the local authority, people using services and their relatives.

Safeguarding

The provider worked with people and healthcare partners to understand what being safe meant to them and the best way to achieve that. Staff concentrated on improving people’s lives while protecting their right to live in safety, free from bullying, harassment, abuse, discrimination, avoidable harm and neglect. The provider shared concerns quickly and appropriately.

 

There were processes in place to support people to be safe from harm. There was a safeguarding policy in place and staff had completed relevant training. There was also a safeguarding tracker in place to log safeguarding concerns and relevant actions taken. People told us they felt safe with staff. One relative told us, “Yes they [staff] are really good, I cannot fault them.”

Involving people to manage risks

The provider worked with people to understand and manage risks by thinking holistically. Staff provided care to meet people’s needs that was safe, supportive and enabled people to do the things that mattered to them.

 

At the time of the assessment, there was no requirement for involvement from other professionals to manage risk. However, leaders and staff had knowledge and systems in place for when this need arose for people using the service. People we spoke with had confidence the provider would involve the appropriate professionals to manage risk, when required.

Safe environments

Score: 3

The provider detected and controlled potential risks in the care environment. They made sure equipment, facilities and technology supported the delivery of safe care.

 

The provider completed environmental checks as part of their initial assessment. People also told us the provider continually assessed environmental risk when completing care calls.

Safe and effective staffing

Score: 3

The provider made sure there were enough qualified, skilled and experienced staff, who received effective support, supervision and development. They worked together well to provide safe care that met people’s individual needs.

 

There were robust systems in place to support safe staff recruitment. Staff had completed relevant training and supervision to fulfil their role. Staff told us they had completed relevant training to support them in their role. One staff member told us, “[The service] are fantastic in terms of training.”

Infection prevention and control

Score: 3

The provider assessed and managed the risk of infection. They detected and controlled the risk of it spreading and shared concerns with appropriate agencies promptly.

 

There were robust systems in place to support safe management of infection. There was an infection prevention and control policy in place. Staff had completed relevant training, and people told us staff wore appropriate personal protective equipment (PPE) when supporting them.

Medicines optimisation

Score: 3

The provider made sure that medicines and treatments were safe and met people’s needs, capacities and preferences. Staff involved people in planning, including when changes happened.

 

At the time of the assessment, the provider did not support people with medication. However, there were robust systems in place for the safe management of medicines. For example, there was a medication policy and relevant training in place. The provider also undertook regular training scenarios regarding medication to ensure they remained competent in administering medications.
