Well-led
Staff knew the service’s vision, aims and objectives and understood their role to achieve this. Leaders were visible and supportive, helping staff develop in their roles. Staff were able to speak up without fear and felt supported to give feedback. Leaders did not always act upon the feedback.
Leaders did not have oversight of the quality of service being delivered. Governance processes were not effective and risk management systems lacked structure and consistency. There was not a clear culture of continuous improvement and leaders did not engage with contractors to monitor and improve service delivery and patient outcomes.
