Patients, and where appropriate, those close to them, were involved in the assessment of their needs. Staff considered the patient’s physical, emotional and personal care needs. Leaders did not always monitor patient outcomes to ensure expectations were met. Staff made sure patients understood their care to enable them to give informed consent.
