Learning culture

Score: 3

The provider had a proactive and positive culture of safety, based on openness and honesty. Staff listened to concerns about safety and took action to address them. Lessons were learnt to continually identify and embed good practice. The registered manager understood their responsibilities under the Duty of Candour and to be open and honest when things went wrong. One person told us, “I am safe and happy with them.” A relative said, “I’m very happy with the carers, they are very good and [Name] is very safe with them.”

Safe systems, pathways and transitions

The provider worked with people and healthcare partners to establish and maintain safe systems of care, in which safety was managed or monitored. Systems were in place to ensure people transitioned to the service in a safe manner.

The registered manager carried out detailed assessments of people’s needs and preferences before they started using the service to ensure their needs could be met. The person, their representatives and professionals were involved. Staff made sure there was continuity of care, such as when people transitioned from hospital. A relative told us, “The care during the transition from hospital to home was second to none.”

Safeguarding

People were safeguarded from abuse. Staff concentrated on improving people’s lives while protecting their right to live in safety, free from bullying, harassment, abuse, discrimination, avoidable harm and neglect. The provider had effective safeguarding systems in place and referrals had been made where appropriate. Staff received regular safeguarding training and knew how to report concerns in a timely manner. A staff member told us, “Whenever we report any issues, they are taken seriously and dealt with promptly and professionally.”

Involving people to manage risks

Risks to people's personal safety and wellbeing were effectively assessed, monitored and managed. Care plans contained detailed risk assessments about people’s individual care and support needs, such as their mobility or dietary requirements.

Staff provided care to meet people’s needs that was safe, supportive and enabled people to do the things that mattered to them. One person said, “I am very happy with the service. I am safe and comfortable with them.”

Safe environments

The provider assessed and monitored potential risks in the environment. People were supported in their own homes and whilst the provider was not responsible for the accommodation, they made sure care plans contained assessments on any obvious risks to people and staff arising in the environment. Any identified environmental risks had been mitigated as far as reasonably possible. A staff member said, “The focus is always on maintaining a safe environment and supporting people to live as independently as possible.”

Safe and effective staffing

The provider made sure there were enough qualified, skilled and experienced staff, who received effective support, supervision, training and development. Staff were recruited safely and appropriate pre-employment checks were completed. Management monitored staff practice through routine observations to ensure they were working to the expected standards.

There was enough staff to meet people’s needs. People confirmed they received care from a consistent staff team, and they were always on time. One person told us, “They are very good, they know how to help me.” A relative said, “[Name] has a small group of staff they know well.”

Infection prevention and control

The provider assessed and managed the risk of infection. They detected and controlled the risk of it spreading and shared concerns with appropriate agencies promptly. Staff were trained in infection control and had access to personal protective equipment. A staff member told us, “Personal protective equipment is always available, which helps keep both staff members and those receiving services safe.”

Medicines optimisation
