Assessing needs

People’s needs were regularly reviewed, and assessments updated to reflect people’s current care and support needs in areas such as, their health, wellbeing and communication. Assessments were person-centred and focused on the individual.

A relative said, “[Registered Manager] came to see us initially and went to visit [Name]. They were very thorough and talked through everything. 4 months ago, we had a review and changed some things. We had a recent reassessment after a hospital admission, and we adjusted times and medication needs. We are always completely involved in planning of [Name’s] care needs.”

Delivering evidence-based care and treatment

The provider planned and delivered people’s care and treatment with them, including what was important and mattered to them. This included providing effective support with eating and drinking where this was part of people’s care package. Appropriate monitoring was in place to ensure people maintained a healthy diet and had sufficient fluids. One person told us, “They help me with my meals, I choose what I want and then they help me to get it all ready.”

How staff, teams and services work together

The provider worked well across teams and services to support people. Staff worked effectively with other professionals to ensure people received effective care, including seeking and acting on advice where needed. People had confidence staff would help if they arrived to find them unwell. People and relatives gave us examples of situations where staff had noticed things of concern and escalated this to the appropriate service. One relative told us, “The service will always contact us or the GP whenever necessary, we completely trust them.”

Supporting people to live healthier lives

The provider supported people to manage their health and wellbeing to maximise their independence, choice and control. Staff supported people to live healthier lives and where possible, reduce their future need for care and support by promoting their independence and wellbeing.

Care plans provided details of what people could independently manage and staff encouraged people to maintain these skills where possible. Referrals to professionals, such as occupational therapists and dieticians had been made where staff identified a decline. A relative said, “These days [Name] doesn’t want to eat but they encourage them.”

Monitoring and improving outcomes

The provider routinely monitored people’s care and treatment to continuously improve it. Records showed that regular reviews took place where people were asked if they were happy with their support and whether it was meeting their needs. Any adjustments needed as a result of these reviews were actioned appropriately. One relative said, “We are always involved in any planning or changes in [Name’s] care.”

The provider told people about their rights around consent and respected these when delivering person-centred care and treatment.

People’s consent to their care was recorded, and people confirmed that their decisions were respected. Where people lacked capacity to make decisions, the principles of the Mental Capacity Act 2005 had been followed. Where others were authorised to make decisions on peoples’ behalf this was clearly recorded. One staff member told us, “I always receive consent consistently throughout calls to complete any tasks.”
