Learning culture

Score: 3

The provider had a proactive and positive culture of safety, based on openness and honesty. Staff listened to concerns about safety and investigated and reported safety events. Lessons were learnt to continually identify and embed good practice.

 

Processes were in place to monitor incidents, accidents, and complaints effectively. The registered manager demonstrated how lessons were learned and shared with staff. Feedback from staff was actively used to drive continuous improvements and embed best practice. Staff reported feeling confident to raise concerns and trusted the registered manager would take appropriate action when needed.

Safe systems, pathways and transitions

The provider worked with people and healthcare partners to establish and maintain safe systems of care, in which safety was managed or monitored. They made sure there was continuity of care, including when people moved between different services.

 

Initial assessments were detailed and involved input from external professionals. The registered manager collaborated with people and their relatives to develop a comprehensive assessment of needs before the care package began. The provider worked closely with external professionals to ensure smooth and seamless transitions into care. One external professional commented, “[The provider] introduced themselves to everyone involved in [person’s] care including the GP surgery, pharmacy, and district nurse, to make sure they all knew who they were and understood their role in [person’s] life.”

Safeguarding

The provider worked with people and healthcare partners to understand what being safe meant to them and the best way to achieve that. Staff concentrated on improving people’s lives while protecting their right to live in safety, free from bullying, harassment, abuse, discrimination, avoidable harm and neglect.

 

The provider shared concerns quickly and appropriately. Staff were up to date with safeguarding training and demonstrated a clear understanding of how to raise concerns. The safeguarding log was comprehensive and showed a strong emphasis on learning from incidents. Relatives told us they felt their family members were safe, with comments such as, “Yes, they are safe, 100%.”

Involving people to manage risks

The provider worked with people to understand and manage risks by thinking holistically. Staff provided care to meet people’s needs that was safe, supportive and enabled people to do the things that mattered to them.

 

Risk assessments were person-centred and tailored to individuals’ needs. The registered manager had established effective systems to ensure risk assessments were regularly reviewed and updated, with active involvement from the people and their relatives.

Safe environments

Score: 3

The provider detected and controlled potential risks in the care environment. They made sure equipment, facilities and technology supported the delivery of safe care.

 

Environmental risks were clearly documented within people’s care plans. These included factors such as property access, cluttered living spaces, and smoking-related hazards. Staff were able to describe practical examples of how they delivered care while maintaining a safe environment. A lone working policy was in place to ensure staff could safely access people’s homes.

Safe and effective staffing

The provider made sure there were enough qualified, skilled and experienced staff, who received effective support, supervision and development. They worked together well to provide safe care that met people’s individual needs.

 

All staff had completed mandatory training and were subject to regular competency checks to ensure best practice. Feedback from relatives confirmed staff demonstrated a high level of knowledge, skill, and professionalism in their roles.

 

Recruitment processes were robust and comprehensive. Detailed pre-employment checks, including verification of identity, references, and suitability, were consistently carried out in line with regulatory requirements.

Infection prevention and control

The provider assessed and managed the risk of infection. They detected and controlled the risk of it spreading and shared concerns with appropriate agencies promptly.

 

Staff demonstrated good knowledge of infection prevention and control (IPC) procedures. Staff said they received appropriate training and received regular spot checks. Staff told us they followed IPC processes to reduce the risk of the spread of infection.

Medicines optimisation

The provider made sure that medicines and treatments were safe and met people’s needs, capacities and preferences. Staff involved people in planning, including when changes happened.

 

Care plans included detailed information about each person’s medicines, ensuring staff had clear guidance on administration and monitoring. Protocols for medicines prescribed on an as-required (PRN) basis were in place and documented. Regular medicine audits were conducted, which identified any issues promptly. Where concerns were found, appropriate actions were taken to address them and improve practice.
