Well-led
Senior managers failed to carry out the necessary actions following the last fire risk assessment in April 2025.
Local leaders were visible, knowledgeable and supportive, helping staff develop in their roles. Governance systems were appropriate for the size of the service and ensured that risks, performance information and clinical outcomes were monitored and acted on. Staff felt supported to give feedback and were treated equally, free from bullying or harassment.

Freedom to speak up
The service fostered a positive culture where people felt they could speak up and their voice would be heard.
Staff we spoke with said they could speak up and that their voice would be heard.
Workforce equality, diversity and inclusion
We did not look at Workforce equality, diversity and inclusion during this assessment. The score for this quality statement is based on the previous rating for Well-led.
Governance, management and sustainability
Senior managers failed to carry out the necessary actions following the last fire risk assessment in April 2025. Whilst the majority of recommendations had been completed there were eight outstanding priority actions, three of which were deemed as high with timescales of completion set at one month from the date of the assessment. We were told these would be included in plans for an extension and upgrade to the building in late 2026. This meant there was a failure to complete the requirements of the assessment.
Local managers had established governance processes that were appropriate for their service. Managers had introduced a recognised electronic knowledge, compliance and workload management tool to support oversight of governance within the practice. Staff could access all required policies and procedures.
We saw managers had undertaken regular audits and had clear responsibilities, roles and systems of accountability.
