We looked for evidence that staff involved people in decisions about their care and treatment and provided them advice and support. Staff regularly reviewed people’s care and worked with other services to achieve this.

Assessing needs

The service made sure people’s care and treatment was effective by assessing and reviewing their health, care, wellbeing and communication needs with them. People using the service completed a detailed history of their health, including travel history, which was reviewed in consultation with the clinician.

Delivering evidence-based care and treatment

Leaders told us clinicians followed legislation and current evidence-based good practice and standards. Systems were in place to ensure staff were up to date with evidence-based guidance and legislation However, there was no evidence of prescribing audits carried out every six months, in line with their medicines management policy. Some of the clinical records we saw showed antibiotic prescribing did not appear to conform to NICE or local microbiological guidance. Following our assessment, the provider submitted an audit on the prescribing of antibiotics for upper respiratory tract infections over a one-year period which demonstrated over 80% of patients issued antibiotics had a laboratory result which supported the use of antibiotics. There were plans to repeat the audit in a years’ time to check if prescribing quality had been maintained.

Feedback received by CQC from several people who used the service indicated they did not understand why the service offered them off label administration, instead of licensed administration, in line with national recommended guidelines. The provider told us patients had a choice between the two. We could not corroborate this with the experience of people who used the service.

Information on consent forms for rabies vaccine made it clear that either intra-muscular or intra-dermal vaccination could be provided. However, feedback from patients we spoke to at one clinic site indicated they were not told it was off label and very little information given during consultation which was less than 5 minutes.

The service did not always tell people about their rights around consent and did not always respect their rights when delivering care and treatment.
