Learning culture

Score: 3

The provider developed a learning culture. Staff reported all accidents, incidents and adverse events. Managers investigated these. They developed action plans to address areas of concern and make improvements at the service. Managers, staff and a relative described a recent incident. They told us this was investigated. They explained staff and the relative were able to contribute their views about how to prevent similar situations in the future. Together they developed a mitigation plan which everyone was aware of. The relative told us this had been well managed, and they felt the person was safe. Staff explained they had been supported to learn from the incident.

Safe systems, pathways and transitions

The provider helped support people to have safe transitions between services. The management team included nurses who understood people’s health and complex care needs. They liaised with other professionals to make sure people received the care they needed from different services.

The provider supported people to transition to their care from other care providers. They assessed people’s needs and made sure they and their representatives were involved in decisions. The provider had offered people’s existing staff teams opportunities to work for them so they could continue to provide continuity of care and support.

Safeguarding

The provider had systems to help safeguard adults and children. Staff undertook training to understand these. Staff were able to explain about the procedures. The provider shared additional guidance and information with staff to help increase their knowledge of how to recognise and report abuse.

People using the service and their relatives told us they were safely cared for. A relative explained, “It feels very safe for [person].”

Involving people to manage risks

The provider ensured risks were managed. They involved people using the service, their relatives and other professionals in planning how to manage risks. For example, when staff identified a potential risk for 1 person, they discussed this with them. With their permission, the provider spoke with other professionals to ensure the equipment the person used was reassessed to make sure this was safe.

The provider assessed risks relating to people’s health and wellbeing. Risk management plans were clear, regularly reviewed and updated when needed.

The provider developed positive strategies and plans to help manage risks associated with people’s anxiety and mental wellbeing. Plans included information about potential triggers and how to avoid these. They also included techniques to help people feel safe and calm. These were personalised and had been developed in collaboration with different professionals and people’s representatives.

Staff completed training to understand how to safely deescalate situations. They were able to tell us about techniques they used. A relative explained, “Staff support [person] well and try to calm down situations. They have had training, and the manager is very strict at monitoring this. We work with staff to teach them what we know.”

Risk assessments were reviewed and updated following incidents.

Safe environments

The provider assessed people’s home environments to make sure they were safe and to identify any risks. They worked with people, their relatives and other professionals to make improvements when these were needed. The registered manager told us they had supported some people on holidays. Staff had assessed the safety of new environments and travelling. They created detailed risk management plans when they identified risks. Relatives told us they were happy with this support.

Safe and effective staffing

There were enough staff to care for people in a safe way and meet their needs. People using the service had an allocated team of staff who supported them. People and their relatives told us the staff knew them well. Their comments included, “Carers arrive on time and stay for the agreed time” and “[Person] has a team of care workers. They are all familiar faces. If a new member of staff joins, they are integrated into the team by experienced staff.”

The provider had suitable procedures for recruiting and selecting staff. These included a range of checks. New staff undertook a range of training. Managers assessed their skills and knowledge.

Staff told us they felt well supported. Staff regularly met with managers to discuss and appraise their work.

Infection prevention and control

The provider had procedures to help prevent and control infection. Staff completed relevant training. Staff were provided with personal protective equipment (PPE) to use when needed. Managers carried out spot checks to observe where staff followed procedures. People using the service and their relatives told us they were happy with staff support in this area. A relative commented, “Staff follow hygiene practices such as wearing gloves. They keep the home environment clean.”

Medicines optimisation

People received their medicines safely and as prescribed. Staff assessed risks relating to people’s medicines. There were clear plans and information to ensure medicines were well managed. Staff worked with doctors to make sure medicines were regularly reviewed when needed. Staff undertook training to understand how to handle medicines. Managers carried out audits of medicines and responded appropriately to any concerns. People using the service and their relatives told us they were happy with the way medicines were managed.
