Assessing needs

Score: 3

The provider made sure people’s care and treatment was effective by assessing and reviewing their health, care, wellbeing and communication needs with them. People’s care plans included relevant information about their health and care needs. People’s communication needs and preferences were documented. Care plans were personalised and updated as people’s needs changed.

Delivering evidence-based care and treatment

Score: 3

The provider planned and delivered people’s care and treatment with them, including what was important and mattered to them. They did this in line with legislation and current evidence-based practice and standards. The service implemented advice from health and social care professionals when required. People’s care plans included relevant details about their health needs, for example allergy information.

How staff, teams and services work together

Score: 3

The provider worked well across teams and services to support people. They made sure people only needed to tell their story once by sharing their assessment of needs when people moved between different services. Staff reported that they worked together well as a team and felt supported by the management team at the service. Staff told us, “I like the people I work with, and the company.”

Supporting people to live healthier lives

Score: 3

The provider supported people to manage their health and wellbeing to maximise their independence, choice and control. Staff told us they felt confident identifying changes in people’s needs. A staff member told us, “If someone’s needs change, we adapt quickly, we let the office know right away.” Records evidenced that feedback from professionals was recorded in care plans.

Monitoring and improving outcomes

Score: 3

The provider routinely monitored people’s care and treatment. People’s care plans were regularly reviewed to ensure they continued to meet people’s needs. Where people’s needs changed the service recorded this. Where people required equipment to meet their needs, the correct referrals were made to support the sourcing of this.

The provider told people about their rights around consent and respected these when delivering person-centred care and treatment. Records evidenced that the managers understood their responsibilities in relation to the Mental Capacity Act (2015) and ensured they undertook assessments of people’s ability to consent to care and treatment. Where people had appointed relatives or friends to act on their behalf this information was clearly documented. Staff understood the need to gain consent from people before supporting people. A person told us, “I decide what I want, yes, they ask for my consent.”
