Person-centred Care
The provider did not always make sure people were at the centre of their care and treatment choices and they did not always work in partnership with people, to decide how to respond to any relevant changes in people’s needs.
Care plans and risk assessment were conflicting and did not fully reflect the persons care needs and how they should be supported and risks mitigated.

Providing Information

Listening to and involving people
The provider did not always make it easy for people to share feedback and ideas, or raise complaints about their care, treatment and support.
The person supported by the agency was able to express their needs, wishes and preferences.A review of records showed care reviews had not been completed providing them with the opportunity to share their views and feedback on the care and support provided.
Other systems referred to key-working sessions, welfare calls and questionnaires. Due to the size of the agency these had yet to be embedded to reflect how people were involved and listened to.
A complaints policy was in place; this was also detailed in the service user guide. We were told no complaints had been received. This was confirmed by the person and their relative who said, “We’ve never needed to raise a concern or complaint.”
