The provider did not always make sure people’s care and treatment were effective because they did not always check and discuss people’s health, care, well-being and communication needs with them.

An initial assessment had been completed with the person and their relative. Information provided detailed the support the person wanted and needed. However, information in the care plan and risk assessment differed. There was no evidence assessed needs had been reviewed and updated to reflect the support provided.

Agency procedures showed and we were told care plans and risk assessment would be reviewed within the first 6 weeks and then formally reviewed every 3 months by a senior manager to check whether if there had been any changes in need. Records seen did not evidence this had been done ensuring the plan remained accurate and effective.

Supporting people to live healthier lives

The provider supported people to manage their health and well-being to maximise their independence, choice and control. Staff supported people to live healthier lives and where possible, reduce their future needs for care and support.

Records showed staff supported the person in managing their personal care, including oral hygiene. Information also included the contact details of health and social care professionals, who could be contacted to share any concerns or where further advice and support could be provided. At the time of the inspection the agency was not working in partnership with other services.

The person's nutritional needs were taken into account, and staff ensured that meals provided were aligned with their cultural diet. This was observed during our visit, where staff provided a traditionalmeal and a drink as requested.

The provider told people about their rights around consent and respected these when delivering person-centred care and treatment.

The care provider and support package had been sourced privately by the person, with support from family. Both had been involved in the assessment and care planning process and said they were happy with the care and support provided. Visit records also showed staff sought consent prior to completing tasks.
