Clarissa's Home Healthcare Services LTD is a homecare provider providing the regulated activities of personal care to people living in their own homes. CQC only inspects where people receive personal care.The registered manager also held the nominated individual role.

At our last assessment we identified 2 breaches of the legal regulations relating to safe care and treatment and good governance. At this inspection, we identified 2 continuing breaches of the legal regulations in safe care and treatment and good governance and an additional breach of the legal regulations relating to fit and proper persons employed.

The provider did not always provide safe care and treatment. Systems and processes were not effective in identifying issues and areas for improvement. There was inconsistent recording within care records. Staff were not always recruited safely. Medicines were not routinely managed safely. Risks relating to people’s care were not always assessed, care records were not always detailed, accurate or person-centred. The provider was not consistently working in line with the Mental Capacity Act 2005 when completing mental capacity assessments. However, the provider worked with professionals and was managing infection prevention and control effectively.

We have asked the provider for an action plan in response to the concerns found at this assessment.

People's experience of the service

People and relatives gave us mixed feedback about the provider. People told us they felt safe and comments included, “[Staff name) is very good. [Staff name] is pleasant and I feel safe with [them]” and, “Yes definitely, it’s the type of people they are. They are caring.” Relatives told us, “Yes [person] seems to feel safe. [Person] seems content and happy”, and, “[staff name] is fantastic, they really get on and [staff name] really does care about [person].” We received mixed feedback about staff training overall. We received mixed feedback about whether staff arrived on time. People and relatives told us staff were polite, friendly, kind, patient, absolutely wonderful and caring. We were told the registered manager listened. People told us, “[staff name] is very caring and ask questions, makes sure I'm ok before [staff name] leaves, [staff name] is just very caring” and “They are nice people really. They have gotten to know me now.” Relatives told us, “They are the best carers we have had by far,” and “Both the [staff names] treat [person] so well and call [person] their [person], we have a laugh and they treat [person] like family.”

People told us they were treated with dignity and respect. Comments included, “Yes, definitely, staff are very nice people. Yes, we chat together and they listen. I have no problems with them at all” and “they [staff] help me get dressed and I can get in a bit of a pickle, but they stay with me and make sure I am ok. They always give me dignity in the shower, they never rush me or are never abrupt, they let me take my time.” Relatives told us, “Yes, they explain all the way through, [person] can be very negative about washing and they calm [person]…” and, “They [staff] think of [person’s] needs and [person’s] dignity first.”
