The service is a residential care home providing accommodation and personal care to people with mental health needs and dementia. There were 4 people living in the home at the time of our assessment. Our assessment was prompted by the length of time since the last assessment.
The provider was previously in breach of the legal regulations in relation safe care and treatment, and good governance and the service was rated requires improvement. Not enough improvements were found at this assessment, and the provider remained in breach of these regulations, and the service has been rated requires improvement again.
People were not always protected from the risk of harm. Information about people’s care plans and risk assessments were not always up to date and accurate. There were shortfalls in care planning and risk assessment processes. We also identified concerns relating to medicines management. This meant people were not being adequately protected from the risk of receiving unsafe and inappropriate care.
The premises were not always maintained to a high standard of cleanliness and the provider had not ensured that adequate handwashing facilities were available in the home to help prevent and control the risk of any infection.
Potential risks to people’s health and welfare had not always been assessed including people’s individual safety and health conditions. People’s care plans did not evidence how they were actively involved in planning for important life changes or discussing their future care needs.
The provider did not have a robust approach to monitoring the effectiveness of people’s care, treatment and support and action was not always taken to continuously improve it.
Governance and quality assurance systems needed improvement to ensure that there was sufficient oversight of the service, as audits had been completed but this had not identified the issues we found during our assessment.
However, staff were caring and attentive to people’s needs. They understood how to keep people safe and how to report concerns. Staff assessed people’s communication needs.
