Safe – this means we looked for evidence that people were protected from abuse and avoidable harm. At our last assessment we rated this key question Requires Improvement. At this assessment the rating has changed to Good. This meant people were safe and protected from avoidable harm.

The provider was previously in breach of the legal regulation in relation to safe care and treatment. Improvements were found at this assessment and the provider was no longer in breach of this regulation.

Learning culture

Score: 3

The provider had a proactive and positive culture of safety, based on openness and honesty. Lessons were learnt to continually identify and embed good practice. The provider had procedures for dealing with incidents, accidents and complaints. The management team investigated these and developed plans for improvements. These were shared with the staff.People received safe care because staff learned from safety alerts and incidents. A staff member commented,“We complete an incident record on the system. I always talk to the manager. If there’s a need, they will always come back to us and discuss.”

Safe systems, pathways and transitions

The provider worked with people and healthcare partners to establish and maintain safe systems of care, in which safety was managed or monitored. They carried out thorough assessments of people’s needs, consulting with the person, their representatives and other professionals involved in their care.Staff made sure there was continuity of care, including when people moved into the service. Information was collected before people started to use the service. One staff member told us, “Information is added promptly to the [care plan] system and can be seen on the handset which everyone has. The system is excellent and works well. We can write notes immediately so it’s always up to date. Any changes are also mentioned at handover.”

Safeguarding

The provider worked with people and healthcare partners to understand what being safe meant to them and the best way to achieve that. Staff concentrated on improving people’s lives while protecting their right to live in safety, free from bullying, harassment, abuse, discrimination, avoidable harm and neglect. Staff had received training in safeguarding people from abuse and were aware of how to raise any concerns. They were confident any concerns raised would be dealt with appropriately. People told us they felt safe living at the home. One person said, “It’s super. They look after us really well. I love it.”

Involving people to manage risks

The provider worked with people to understand and manage risks by thinking holistically. Staff provided care to meet people’s needs that was safe, supportive and enabled people to do the things that mattered to them. People were involved in decisions about risk. Staff understood how to support people in a way that ensured risks were managed, whilst respecting people’s choices and individuality. Risks were assessed and any identified risk was detailed in care plans. Care plans contained guidance for staff to follow to keep people safe, including how to respond when people became distressed.

Safe environments

The provider detected and controlled potential risks in the care environment. They made sure equipment, facilities and technology supported the delivery of safe care. There was a programme of redecoration to ensure the environment remained in good condition. People were able to bring their own belongings to decorate their rooms.There was a large and accessible garden with an area of seating. The provider ensured people had the equipment they needed to stay safe. This included equipment to help them move, sensors to alert staff if people fell and adjustable beds.The provider aimed to support people to remain independent whenever possible.One relative told us, “It’s small, homely and with an attention to detail. They fit to individual needs”.

Safe and effective staffing

The provider made sure there were enough qualified, skilled and experienced staff, who received effective support, supervision and development. They worked together well to provide safe care that met people’s individual needs. The provider had a robust recruitment policy which showed pre-employment checks were carried out to ensure appropriate staff were selected. The provider had systems in place to determine staffing levels. We observed care and support that was provided promptly, but in an unrushed way.

Infection prevention and control

The provider assessed and managed the risk of infection. They detected and controlled the risk of it spreading and shared concerns with appropriate agencies promptly.The home and equipment were clean, and risks associated with infections were assessed and well managed. There were regular checks and audits of cleanliness and hygiene. Staff had received training in infection prevention and control. One staff member told us, “Infection control is good. Everywhere is kept clean and we know good practice, like making sure people’s hands are washed before eating.”

Medicines optimisation

The provider made sure that medicines and treatments were safe and met people’s needs, capacities and preferences. Processes were in place to ensure people received their medicines as prescribed. Medicines were administered and stored safely. Some people were prescribed medicines on an ‘as and when’ required basis, often referred to as PRN. Protocols were in place to ensure people received PRN medicines appropriately.
