At our last assessment we rated this key question good. At this assessment the rating has remained good. This meant people’s outcomes were consistently good, and people’s feedback confirmed this.

Assessing needs

The provider made sure people’s care and treatment was effective by assessing and reviewing their health, care, wellbeing and communication needs with them. The registered manager showed a clear process of assessment when a person planned to move into the home. People’s care planning documents were reviewed on a regular basis and when required, hospital passports were generated from this information. Staff understood people’s needs and wishes, however not all of the records relating to care and support delivered to people were accurate. The registered manager had identified this and was taking action to make improvements. Family members told us they were involved in the planning of their relative’s care and support.

Monitoring and improving outcomes

The provider routinely monitored people’s care and treatment to continuously improve it. They ensured that outcomes were positive and consistent, and that they met both clinical expectations and the expectations of people themselves. People were supported by local district nursing team to meet people’s clinical needs. When required, referrals were made to specialist local health teams to support people. For example, the later life mental health team. Family members told us they were involved in the relatives care and support. One family member told us “Continually checks on (relatives) care and asks the staff countless questions”, staff are always “kind and responsive."

The provider told people about their rights around consent and respected these when delivering person-centred care and treatment. People were supported in accordance with the Mental Capacity Act. Care plans contained information relating to decision making. Staff received training around the Mental Capacity Act of Deprivation of Liberty safeguards. We observed staff offering people choices and support to make their own decisions.
