Well-led – this means we looked for evidence that service leadership, management and governance assured high-quality, person-centred care; supported learning and innovation; and promoted an open, fair culture.

At our last assessment we rated this key question requires improvement. At this assessment the rating has remained requires improvement.

This meant the management and leadership was inconsistent. Leaders and the culture they created did not always support the delivery of high-quality, person-centred care. The service was in breach of legal regulation in relation to governance at the service.

Shared direction and culture

Capable, compassionate and inclusive leaders

Score: 2

The provider was driving changes to strengthen accountability and oversight. Records were promptly provided during the assessment, and engagement with the process was positive, with the registered manager and management support functions welcoming feedback and viewing it as an opportunity for improvement. Clear plans were in place to ensure responsibilities and roles were understood and governance systems became more effective. Internal and provider-level audits had been strengthened, but their effectiveness still needed improvement to consistently identify shortfalls. We identified breaches of regulation relating to medicines, risk, and governance, and whilst good progress had been made, further work was required.

Governance, management and sustainability

Score: 2

Leadership at the service had improved, and the registered manager was supported by team leaders in driving changes. Audits had been strengthened, but their effectiveness still needed improvement to ensure all shortfalls were identified. The provider’s internal teams, such as quality and health and safety, continued to support the registered manager, and this support was needed to ensure sustainability long term. Monitoring from the local authority was also in place, helping to drive improvements. They acknowledged that progress had been made and confirmed further work was required.

Learning, improvement and innovation

Score: 3

The provider demonstrated a stronger commitment to continuous learning and improvement, taking a lessons-learned approach to drive better outcomes for people. Engagement with the assessment process was positive, and the registered manager and regional manager welcomed feedback as an opportunity for growth. An on-going action plan was in place, and work had begun to address key areas, with further improvements being added to ensure all areas identified during the assessment were detailed with clear actions and outcomes. Staff were supported through training, supervision, and regular meetings, and they reported confidence in the support provided and improvements made. Feedback surveys from people and relatives showed they were happy with the care and support they received, and opportunities were provided for them to share their views to inform service improvements.
