At our last assessment we rated this key question requires improvement. At this assessment the rating has remained requires improvement. This meant people’s needs were not always met.

Person-centred Care
The provider made sure people were at the centre of their care and treatment choices and they decided, in partnership with people, how to respond to any relevant changes in people’s needs. The registered manager and staff team saw people were at the heart of their care, and staff were mindful that they were working in people’s homes and did so with respect. The provider was driving improvements to ensure people remained central to their care and treatment choices. Re-decoration had been recognised as necessary, and work was underway to update spaces and make them more homely. People were involved in a range of activities of their choice, supporting a sense of inclusion and wellbeing.
Care provision, Integration and continuity
The provider understood the diverse health and care needs of people and their local communities, so care was joined-up, flexible and supported choice and continuity. People were increasingly at the centre of their care, and improvements had been made to better understand and support their diverse health and care needs. Staff worked with external professionals and made referrals for people, such as SALT and psychology, and some guidance had been incorporated into care planning. The provider was driving improvements to ensure care was joined-up, flexible, and supportive of choice and continuity. However, further work was needed to strengthen recording of professional advice in care plans. Where shortfalls in care planning were identified, the provider immediately updated records to ensure they were more detailed and up to date. Reviewing information was ongoing, and this was included as an action within the provider’s improvement plan.

Equity in access
The provider made sure that people could access the care, support and treatment they needed when they needed it. Improvements had been made to help people access the care, support, and treatment they needed, and staff were more responsive and reactive to people’s needs. Through improvements in reporting and recording, timely access to care was now being regularly identified. Audits had been strengthened to support these improvements, and they required embedding into practice.
