Learning culture

Score: 3

The provider had a proactive and positive culture of safety, based on openness and honesty. Staff listened to concerns about safety and investigated and reported safety events. Lessons were learnt to continually identify and embed good practice. The provider shared lessons learned from other services as well as their own to ensure their staff learned from a variety of scenarios. These also included examples of good practice that they wanted to embed among their staff.

Safe systems, pathways and transitions

The provider worked with people and healthcare partners to establish and maintain safe systems of care, in which safety was managed or monitored. People’s care plans reflected the joined-up care that had been agreed with healthcare partners. The provider made sure there was continuity of care, including when people moved between different services and information about people’s needs was shared with all involved. Staff were aware of the different professionals involved in people’s care and knew who to contact if they had any concerns.

Safeguarding

The provider worked with people and healthcare partners to understand what being safe meant to them and the best way to achieve that. Staff concentrated on improving people’s lives while protecting their right to live in safety, free from bullying, harassment, abuse, discrimination, avoidable harm and neglect. Staff had been trained in safeguarding procedures and demonstrated a good understanding of their responsibilities. The provider shared concerns quickly and appropriately as required and had a clear safeguarding policy and procedure in place.

Involving people to manage risks

The provider worked with people to understand and manage risks by thinking holistically. Staff provided care to meet people’s needs that was safe, supportive and enabled people to do the things that mattered to them. The provider had clear risk assessments in place to support staff in their management of risks to people’s health and safety. The provider worked with people to manage risks they wanted to take, for example, in conducting activities. There was an on-call system in place for the management of emergencies, which included senior management who were on-call to provide additional support if needed.

Safe environments

The provider detected and controlled potential risks in people’s home environments. They conducted environmental risk assessments to ensure facilities supported the delivery of care. Risk assessments were thorough and covered different areas of people’s properties. Where issues were identified, the provider recorded the issue and took action to rectify these.

Safe and effective staffing

Score: 3

The provider made sure there were enough qualified, skilled and experienced staff, who received effective support, supervision and development. There were professional development opportunities available to staff as well as clear performance management procedures if needed. They worked together well to provide safe care that met people’s individual needs.

The provider conducted a thorough induction with staff, and they confirmed they found this useful. One staff member told us, “The induction was excellent. It really prepared me for the job.” Thereafter quarterly supervisions, spot checks and informal one to ones were conducted with staff to ensure they were supported. If any issues were identified, these were discussed with staff and action was taken to support them to rectify these.

The provider worked closely with their internal Occupational Health team as well as Human Resources to ensure any adjustments that were needed with staff’s roles took place and any disciplinary or performance management issues were handled fairly.

Infection prevention and control

The provider assessed and managed the risk of infection. They detected and controlled the risk of it spreading and shared concerns with appropriate agencies promptly. There was a clear policy and procedure in place and staff had received training. Individual potential infection risks were identified and clear risk management plans were in place. People told us staff were clean and tidy when conducting their roles. One person told us “They are very clean. They use gloves and aprons and keep the place very tidy.”

Medicines optimisation

The provider made sure that medicines were safe and met people’s needs, capacities and preferences. Staff had received training in medicines management and there were clear risk assessments and medicines administration records (MARs) in place to evidence what medicines people had taken. The provider had a clear medicines administration policy and procedure in place which was up to date and clearly stipulated staff responsibilities.
