Effective – this means we looked for evidence that people’s care, treatment and support achieved good outcomes and promoted a good quality of life, based on best available evidence. At our last assessment we rated this key question good. At this assessment the rating has remained good. This meant people’s outcomes were consistently good, and people’s feedback confirmed this.

Assessing needs

Score: 3

The provider made sure people’s care and treatment was effective by assessing and reviewing their health, care, wellbeing and communication needs with them. People’s needs were assessed prior to them starting to live at the home. Assessments were detailed and covered peoples care and support needs and what was important to and for each person.

Delivering evidence-based care and treatment

Score: 3

The provider planned and delivered people’s care and treatment with them, including what was important and mattered to them. They did this in line with legislation and current evidence-based good practice and standards.

People’s care and treatment were regularly reviewed, and staff sought specialist advice where needed. Staff supported people with nutrition and hydration. Most people told us they enjoyed the food, but some people told us mealtimes could be arranged differently, so that breakfast and the days main meal, which was served at lunch time, were more spaced. Records showed mealtimes had recently been discussed at residents’ meetings. The provider said they would review the lunch time service, including seeking people’s views again.

How staff, teams and services work together

The provider worked well across teams and services to support people. They made sure people only needed to tell their story once by sharing their assessment of needs when people moved between different services.

Managers and staff collaborated and communicated effectively with each other, and external health and social care professionals.

Supporting people to live healthier lives

The provider supported people to manage their health and well-being to maximise their independence, choice and control. Staff supported people to live healthier lives and where possible, reduce their future needs for care and support.

People received support around nutrition, hydration, lifestyle choices and emotional well-being. Care records gave staff detailed information about people’s health conditions, how these might affect the person and when staff should raise concerns.

Monitoring and improving outcomes

The provider routinely monitored people’s care and treatment to continuously improve it. They ensured that outcomes were positive and consistent, and that they met both clinical expectations and the expectations of people themselves.

There was regular review of peoples care and support needs. When changes occurred, staff ensured action was taken and any changes were reflected in people’s records.

The provider told people about their rights around consent and respected these when delivering person-centred care and treatment.

The provider was meeting the requirements of the Mental Capacity Act 2005 (MCA). Care records included evidence of people’s consent had been sought, including assessments of people’s capacity, and best interest decisions. Some care records had contradictory older information regarding consent or were not clear about what consent had been sought for. Managers were in the process of auditing records relating to capacity and consent to ensure clarity and remove out of date information. People told us staff involved them in all decisions about their care.
