Learning culture

Score: 3

The provider had a proactive and positive culture of safety, based on openness and honesty. Staff listened to concerns about safety and investigated and reported safety events. Lessons were learnt to continually identify and embed good practice.

There were systems and processes ensuring that lessons were learned following accidents and incidents. This included a social media channel for care workers to share any concerns, regular team meetings to discuss accidents and incidents and a log to record actions taken.

Care workers told us that they would speak to the registered manager and report any concerns and felt confident that the registered manager would take the appropriate actions to improve care and support for people who used the service. A care worker told us, “Yes, there is always a lesson to learn when anything goes wrong. However, we always discuss any issues with the manager.”

 

Safe systems, pathways and transitions

The provider worked with people and healthcare partners to establish and maintain safe systems of care, in which safety was managed or monitored. They made sure there was continuity of care, including when people moved between different services.

There were clear processes to report and share care and health related issues with external health care partners. The registered manager advised us that they would get in touch with health and social care partners to ensure people’s needs were fully met. The registered manager said, “If clients require any additional support or their condition would deteriorate, I would speak to their social workers, GP or any other relevant health care professionals.”

 

Safeguarding

The provider worked with people and healthcare partners to understand what being safe meant to them and the best way to achieve that. Staff concentrated on improving people’s lives while protecting their right to live in safety, free from bullying, harassment, abuse, discrimination, avoidable harm and neglect. The provider shared concerns quickly and appropriately.

We found robust systems and procedures to respond and deal with any issues where people had or had potentially come to harm. Care workers demonstrated good understanding of how and when to report safeguarding concerns and had received the relevant training. A care worker said, “I would report safeguarding to manager or social worker. I am confident the manager would deal with it.”

Relatives told us that their loved one was safe with care staff. A relative said, “Yes, my [relative] is safe with carers, they are very good.”

Involving people to manage risks

The provider worked with people to understand and manage risks by thinking holistically. Staff provided care to meet people’s needs that was safe, supportive and enabled people to do the things that mattered to them.

We saw robust risk assessments and risk management plans which were reviewed and updated regularly or once people’s needs changed. People and relatives were involved in this process. A relative said, “I meet the registered manager regularly to discuss if anything needs to be changed. I feel I am fully involved.”

Safe environments

The provider detected and controlled potential risks in the care environment. They made sure equipment, facilities and technology supported the delivery of safe care.

Part of the initial assessment was an assessment of the environment, this included establishing any trip hazards or areas within the persons home which presented a risk when providing the regulated activity of personal care.

Safe and effective staffing

The provider made sure there were enough qualified, skilled and experienced staff, who received effective support, supervision and development. They worked together well to provide safe care that met people’s individual needs.

The provider employed enough staff to meet people’s needs and keep them safe. The same familiar staff cared for people, and they developed positive relationships. Relatives told us care workers arrived on time and stayed for the agreed length of time or longer. A relative said, “We have the same carers looking after my relative. They have a good relationship and work well together.”

The provider had systems for recruiting staff to make sure they were suitable and had the skills needed. They carried out interviews, checks on their suitability and provided a thorough induction. Their recruitment processes included checking staff skills, qualifications and right to work in the United Kingdom. The registered manager said, “We do background checks on staff, Disclosure and Barring (DBS) checks, references, we have interview process in place and do panel interviews.” This was confirmed by care workers we spoke with.

Infection prevention and control

The provider assessed and managed the risk of infection. They detected and controlled the risk of it spreading and shared concerns with appropriate agencies promptly.

The service had an infection control procedure and care workers had received appropriate training to ensure people were safeguarded and protected from infections.

Sufficient personal protective equipment (PPE) was available, and relatives observed that care workers followed appropriate infection control procedures. A relative said, “Carers wash their hands and leave the home clean and tidy.”

Medicines optimisation

The provider made sure that medicines and treatments were safe and met people’s needs, capacities and preferences. Staff involved people in planning, including when changes happened.

Care workers and the registered manager involved people and relatives in planning, including when changes happened. Care workers received medicine training when they started in their role. The provider checked staff competence annually. Medicine audits showed medicines were administered in accordance with people’s care plans. Audits were carried out to check the quality of care this included medicine audits. This meant people could be assured that their medicines were administered safely.
