Person-centred Care

The provider made sure people were at the centre of their care and treatment choices and they decided, in partnership with people, how to respond to any relevant changes in people’s needs.
Although care records were not always person-centred, people and relatives told us that the care people received was person-centred.

Care provision, Integration and continuity

Score: 3

The provider understood the diverse health and care needs of people and their local communities, so care was joined-up, flexible and supported choice and continuity. People currently receiving care did not require support with accessing other health and social care providers. The registered manager was aware of their responsibilities. Robust handovers were provided to families and other care company’s staff when required.

Providing Information

The provider supplied appropriate, accurate and up-to-date information in formats that were tailored to individual needs.
People and relatives raised no concerns around communication. On relative said, “[Staff] communication is very good.”
 

Listening to and involving people

The provider made it easy for people to share feedback and ideas, or raise complaints about their care, treatment and support. Staff involved people in decisions about their care.
The management team regularly spoke to people or their advocates to check they were happy with the care they received. When complaints had been raised, the staff had acted on them quickly however, they hadn’t always been recorded in line with their complaints policy.
 

Equity in access

The provider made sure that people could access the care, support and treatment they needed when they needed it. Staff understood the importance of ensuring people had access to the right health and social care services.

Equity in experiences and outcomes

Staff and leaders actively listened to information about people who are most likely to experience inequality in experience or outcomes and tailored their care, support and treatment in response to this. The registered manages ensured staff understood people’s needs and preferences; working with families they put the right processes in place to support people and staff.

Planning for the future

Score: 3

At the time of the inspection no one was receiving end of life care. Staff had received suitable training and guidance was in place if needed.
