Assessing needs

Score: 3

The service made sure people’s care and treatment was effective by assessing and reviewing their health, care, wellbeing and communication needs with them.

People’s needs were assessed holistically, incorporating physical and emotional wellbeing. Care plans were person centred and representative of people’s individual needs, likes and preferences. Consideration had been given to all aspects of people’s lives in care planning. For example, use of different communication aids for those with communication difficulties. People told us they were involved in care planning. One person told us, “Yes, I have a care plan, and I was involved in writing it at the beginning.” Preadmission assessments were thoroughly completed and promoted input from people as well as professionals.

The service told people about their rights around consent and respected these when delivering person-centred care and treatment.

Consent to care assessments had been completed for people and took consideration of the principles of the MCA (2005). Where people lacked capacity, the principles of the MCA (2005) were followed. Capacity assessments were completed, and best interest decisions were in place. People’s consent was sought before any interventions were carried out by staff. Consent to care was regularly reviewed with people when their needs changed.
