Assessing needs

The provider made sure people’s care and treatment was effective by assessing and reviewing their health, care, wellbeing and communication needs with them. We found care records had been reviewed. The provider told us they involved people and their families when assessing needs of people and they updated care plans and risk assessments when there were changes in people's needs and if concerns were identified.

Delivering evidence-based care and treatment

The provider planned and delivered people’s care and treatment with them, including what was important and mattered to them. They did this in line with legislation and current evidence-based good practice and standards. Relatives told us staff were aware of people's dietary needs and preferences. Care records included guidance for staff on how to support people with nutrition and hydration. The provider told us they would work with professionals involved in people's care, such as, occupational therapists.

How staff, teams and services work together

The provider worked well across teams and services to support people. They made sure people only needed to tell their story once by sharing their assessment of needs when people moved between different services. The provider told us they empowered their staff to make connections with professionals and staff were trusted to make these connections and feedback to the provider.

Supporting people to live healthier lives

The provider supported people to manage their health and wellbeing to maximise their independence, choice and control. Staff supported people to live healthier lives and where possible, reduce their future needs for care and support. The provider told us they supported people by communicating with families and changes in people’s health. For example, requesting updates on people's mobility needs.

Monitoring and improving outcomes

The provider routinely monitored people’s care and treatment to continuously improve it. They ensured that outcomes were positive and consistent, and that they met both clinical expectations and the expectations of people themselves. The provider told us they would monitor any changes, for example, wound care, report and record this, work with professionals such as the nurse and follow the advice given. When monitoring people's health and referring them to health care services, relatives told us, "Staff offered help."

The provider told people about their rights around consent and respected these when delivering person-centred care and treatment. The provider told us staff communicated with people so people were involved in their care and were given choice and staff would keep families updated with handovers. Relatives told us staff asked for consent before taking any action. Comments included, "Staff ask" and consent was asked for "Personal care and food."
