Effective – this means we looked for evidence that people’s care, treatment and support achieved good outcomes and promoted a good quality of life, based on best available evidence. At our last assessment we rated this key question inadequate. At this assessment the rating has remained the same due to our inspection activity only looking at specific quality statements.

The provider had improved their processes for monitoring wounds and bruising, ensuring more timely identification, documentation and follow up care. We spent some time with nursing staff going through the services internal processes on how they identify, record and report wounds and bruising. Whilst we found these processes had improved, we noted there was still a high number of bruising being recorded. We discussed this with the area manager who told us they had also identified this and were mentoring nursing staff to ensure they were recording correctly. They had identified some of the recorded bruising was not required, a nursing staff member also confirmed this. The area manager told us they would be monitoring and reviewing this process. Staff had recently completed wound care training and were also due to complete further face to face training with external professionals. Due to the timescale of our inspections, more was time was needed to ensure these processes were embedded and effective.
