Safe – this means we looked for evidence that people were protected from abuse and avoidable harm. At our last assessment we rated this key question requires improvement. At this assessment the rating has changed to good. This meant people were safe and protected from avoidable harm.

Learning culture

Score: 3

The provider had a proactive and positive culture of safety, based on openness and honesty. Staff listened to concerns about safety and investigated and reported safety events. Lessons were learnt to continually identify and embed good practice.

Staff told us they felt confident to raise any issues or concerns to leaders and they knew they would be listened to. The registered manager created a positive culture of openness and transparency. Where there had been an adverse incident, this was investigated and learned from to minimise the risk of it happening again.

Safe systems, pathways and transitions

The provider worked with people and healthcare partners to establish and maintain safe systems of care, in which safety was managed or monitored. They made sure there was continuity of care, including when people moved between different services.

Staff and leaders had a good working relationship with partner organisations such as the local hospital. The service worked closely with healthcare partners to ensure people left hospital when they were ready without delay. Staff knew people well and were able to share information about the person with confidence.

Safeguarding

The provider worked with people and healthcare partners to understand what being safe meant to them and the best way to achieve that. Staff concentrated on improving people’s lives while protecting their right to live in safety, free from bullying, harassment, abuse, discrimination, avoidable harm and neglect. The provider shared concerns quickly and appropriately.

People were protected from the risk of abuse or neglect. Staff were trained in safeguarding adults and children, and staff had a good understanding of how to escalate concerns. Leaders worked closely with the Local Authority in sharing safeguarding concerns. One relative told us, “We all feel safe when the carers are here they are amazing.”

Involving people to manage risks

The provider worked with people to understand and manage risks by thinking holistically. Staff provided care to meet people’s needs that was safe, supportive and enabled people to do the things that mattered to them.

Risk assessments were person specific and focused on how the person would like to manage their risks and maintain their safety. People told us they felt included in decisions about their care and management of risks. One relative told us, “Safety was the key aspect of deciding who would look after my spouse. He had been treated rather poorly with the previous provider which is why I wanted the best and this is it.” Staff regularly rang people to review their care and risks to understand if there were any concerns the person had.

Safe environments

The provider detected and controlled potential risks in the care environment. They made sure equipment, facilities and technology supported the delivery of safe care.

The provider completed environmental risk assessments of people’s homes to make sure it was safe for staff to provide support. Staff and leaders worked with people and their relatives to resolve any environmental issues which could impact the person’s safety and any outstanding risks were clearly documented for staff to be aware of.

Safe and effective staffing

The provider made sure there were enough qualified, skilled and experienced staff, who received effective support, supervision and development. They worked together well to provide safe care that met people’s individual needs.

People told us they received support on time by the same staff team. One person told us, “I like the fact that usually, apart from holidays, the same carers attend. This increases confidence and allows relationships to be formed.” Staff felt they had the skills necessary to complete their role. All staff had completed the necessary training and the registered manager had a proactive system to monitor when staff needed to renew their training.

Infection prevention and control

The provider assessed and managed the risk of infection. They detected and controlled the risk of it spreading and shared concerns with appropriate agencies promptly.

Through a combination of spot checks, conversations with people and staff training, the provider ensured people were protected from the risk of infection transmission. People told us staff always wore personal protective equipment (PPE).

Medicines optimisation

The provider made sure that medicines and treatments were safe and met people’s needs, capacities and preferences. Staff involved people in planning, including when changes happened.

Staff were skilled and trained in managing medication. One member of staff told us, “The training we receive is very good. It gave me the skills I needed.” Leaders ensured staff had clear guidance on what medication needed to be given when. People felt included and consulted about the management of their medication and staff knew what they were doing. One relative told us, “I trust the staff to administer the right medicine for my [relative] and they always make sure they have taken the tablets.”
