Effective – this means we looked for evidence that people’s care, treatment and support achieved good outcomes and promoted a good quality of life, based on best available evidence. At our last assessment we rated this key question good. At this assessment the rating has remained good. This meant people’s outcomes were consistently good, and people’s feedback confirmed this.

Assessing needs

The provider made sure people’s care and treatment was effective by assessing and reviewing their health, care, wellbeing and communication needs with them.

People had up to date, regularly reviewed and person centred care plans. These accurately reflected people’s situations and guided staff on people’s wishes and preferences. Staff knew people well so changes and deteriorations were identified promptly so medical assistance could be sought.

Delivering evidence-based care and treatment

How staff, teams and services work together

The provider worked well across teams and services to support people. They made sure people only needed to tell their story once by sharing their assessment of needs when people moved between different services. Staff knew people well and they worked across the different staff teams and communicated with visiting professionals well.

Supporting people to live healthier lives

The provider supported people to manage their health and wellbeing to maximise their independence, choice and control. Staff supported people to live healthier lives and where possible, reduce their future needs for care and support.

There were a wide variety of activities taking place which people enjoyed. There was a clear timetable of activities taking place every day of the week and people living in different parts of the service were supported to access communal activities downstairs. There were regular visits from the GP and staff were prepared in advanced to ensure people who were unwell saw the GP.

Monitoring and improving outcomes

The provider routinely monitored people’s care and treatment to continuously improve it. They ensured that outcomes were positive and consistent, and that they met both clinical expectations and the expectations of people themselves.

Staff completed regular assessments to monitor people’s weight, skin breakdown, malnutrition risk and weight loss risks. We did find some people did not have positional changes in line with their care plans. There were daily meetings to review changes and risk people may experience to ensure changes were responded to in a timely way. This ensured people health was monitored to ensure their wellbeing was maintained.

The provider told people about their rights around consent and respected these when delivering person-centred care and treatment.

People gave informed consent to their care and treatment. Consent forms were in place and for those who were unable to give informed consent, mental capacity assessments and best interests decisions were in place. Staff understood how to care for people living with dementia however 1 relative felt additional training would be beneficial, so staff understood different types of dementia.
