We reviewed care provision, integration and continuity quality statement for the responsive key question. The service worked well with partners such as the local trust, GP’s, the Integrated Care Board (ICB) and district nursing teams. There were weekly multi disciplinary meetings between the local trust and the service to discuss the care plans of patients. Patients and family members we spoke with confirmed that they saw the same person and that they were reliable, knowledgeable and caring. The views of people who used the service, partners and staff are listened to and taken into account.

Care provision, Integration and continuity

The service had an 7 person inpatient unit for when service users required monitoring for a change in their treatment or for when they were end-of-life. The inpatient unit used a tool that measured the acuity of its service users to ensure that the unit was adequately staffed and only admitted patients when it was safe to do so. This meant that the service did not always have a full bed base. For example, between April and June 2023, the number of occupied beds of the inpatient unit was 67.5% against an industry average of 77.4%. The service was partly funded by the ICB and by charitable fundraising efforts.

The service provided care to people in their own homes with a team of Palliative Care Clinical Nurse Specialists and a Hospice to Home team which enabled people to be treated or die at home if this was their preferred place of dying. People we spoke with said care was joined up, flexible and supported choice and continuity.

The service understood the diverse health and care needs of people and its local community. Care was joined up between the different organisations and was flexible and supported choice and continuity. The service worked with local organisations and the ICB to improve the quality of care provided to patients.
