We reviewed monitoring and improving outcomes quality statement for the effective key question. We found the service monitored people care and treatment via regular audits and accreditation schemes and used this information to improve the care provided to patients at the end of their life.

Monitoring and improving outcomes

The service carried out regular audits such as infection prevention control, hand hygiene, medicine management and documentation audits. These showed good levels of compliance with any learning from the audits shared with the relevant teams.

The service reported to the Integrated Care Board on measures such as the number of referrals, the number of admissions and the average length of stay for patients. It used benchmarking data from Hospice UK in order to measure its service. Hospice UK is the national charity for hospice and end-of-life care. The service had worked towards several accreditation schemes for example, all chefs were trained on the International Dsyphagia Diet Standardisation Initiative framework. This was a framework which empowers all patients to enjoy food no matter what level was their ability to swallow.
