Effective
The provider had a positive reporting culture and staff understood their role in reporting incident. Learning from incidents was shared proactively to become a learning opportunity for all staff. The provider demonstrated the importance of a culture of improvement. Staff felt listened to and engaged in feedback sessions. The needs of children and young people were fully assessed and understood by staff and relatives. The service made sure staff were competent for their roles. Managers appraised staff’s performance and supervisions to provide support and development. The service provides practical support and advice to lead healthier lives. Staff had access to up-to-date, accurate and comprehensive information on children and young people’s care and treatment. All staff had access to an electronic records system that they could all update.

Assessing needs
Families told us they were confident the needs of their young people were fully assessed and understood by the teams providing care. Families reflected they felt heard and valued by staff and they took the time to get to know the young people in their care.
Staff undertook regular assessments, depending on the needs of the young person. Occupational therapists and nurses undertook joint assessments to review changing needs and suitability of current equipment or aids. Staff understood the process of creating care plans that met the needs of children and families. Staff felt the handover process to keep them up to date when a person’s needs changed was robust.
Staff made sure people’s care and treatment was effective by assessing and reviewing their health, care, wellbeing and communication needs with them regularly . These assessments enabled staff to make appropriate amendments to care plans to reflect the needs of the young person. The system flagged to staff when an assessment was due. We reviewed care plans which were detailed with appropriate action taken and clear communication documented.

How staff, teams and services work together
Families felt different parts of the care their young person received worked well together. They felt they were kept up to date with health care arrangements by being included in multi-disciplinary meetings. When they were unable to attend information was shared with them by staff.
Staff understood the significance of effective communication and could see the importance of their role. Staff had access to duty clinicians, safeguarding nurses and other roles, for high level input and additional support. Staff described collaborative working between professionals to make pathways of care seamless and streamlined.
We did not review this as part of the assessment.
Staff used processes which supported collaborative working. Care plans were clear and contemporaneous giving staff an accurate understanding of a person’s needs. Staff continued using their own processes to ensure the young person was still supported when they moved to another local authority. This was to maintain their safety and access to care until a service was provided in the new area. There were effective multi-disciplinary meetings to discuss the needs of young people using their service. These were well recorded and demonstrated a collaborative working relationship in support of the child.
