Terms of this registration relating to carrying out this regulated activity

The registered provider must ensure that the regulated activity maternity and midwifery services is managed by an individual who is registered as a manager in respect of that activity at or from all locations.

The registered provider must ensure that the regulated activity family planning is managed by an individual who is registered as a manager in respect of that activity at or from all locations.

Treatment of disease, disorder or injury

Condition of this registration relating to carrying out this regulated activity

The provider must engage with all support packages offered by external organisations such as (but not limited to) NHS England, NHS Dorset ICB, Primary Care Network and Local Medical Committee.

The registered provider must not register any new service users at The Old Dispensary without the written permission of the Care Quality Commission unless those service users are newly born babies, newly fostered or adopted children of service users already registered with the practice.

The registered provider must ensure all service users registered at The Old Dispensary with long-term and/or chronic conditions (including but not limited to diabetes, asthma, COPD, heart failure, hypothyroidism, depression, mental health, atrial fibrillation and hypertension) have received a monitoring review of their care and treatment by 12pm on 1 November 2022

a) The provider shall provide the Care Quality Commission with an action plan by 9am 17 August 2022 to include:
i. the resource identified to undertake the actions and consultations required for service users identified within the review;
ii. identification of service users for prioritisation of review and detail of the process used by utilising relevant searches within your clinical patient records system;
iii. the total number of service users requiring a review, which includes consultation with and appropriate monitoring care or treatment for each service user, within each long-term condition group;
iv. how the reviews will be risk assessed, so they can be undertaken in order of priority;
v. From 26 August 2022, and every other Friday by 12pm thereafter, the provider must provide to the Care Quality Commission with an update of progress made against the action plan.

The registered provider must provide the Care Quality Commission with an action plan by 9am 17 August 2022 to include actions they intend to take to implement a sustainable system to ensure all service users receive a medicines review within each 12-month period.

Medicines reviews for all those service users identified as not having had a medicines review within the last 12 months must be completed, to include; prescribing in line with current national guidance; consultation with and up to date appropriate monitoring for each service user, by 12pm on 1 November 2022.
a) The action plan submitted to us by 9am on 17 August 2022 must detail;
i. how the reviews will be risk assessed, so they can be undertaken in order of priority.
ii. the resource identified to undertake the actions and consultations required for service users identified for medication review within the audit;
b) From 26 August 2022, and every other Friday thereafter by 12pm, the provider must provide to the Care Quality Commission with an update of progress made against the action plan.

The registered provider must by 9am on 17 August 2022 inform the Care
Quality Commission of what actions they intend to take to implement a
sustainable system to ensure Medicines and Healthcare products Regulatory
Agency (MHRA) drug safety updates will be received and actioned in a timely
and appropriate manner.
a) The action plan must demonstrate who will have assigned responsibility for the receipt and actioning of each update henceforth.
b) The registered provider must undertake a review of historical MHRA
drug safety updates from 1 April 2014 and provide Care Quality
Commission with a copy of the review together with the registered
provider’s actions as appropriate by 9am on 1 November 2022 after
completion of the review.

The registered provider must by 9am on 17 August 2022 provide the Care Quality Commission with an action plan detailing what actions they intend to take to implement a strengthened and sustainable operational clinical and managerial leadership at The Old Dispensary. This is to ensure effective governance processes and improvement plans are implemented, which will address the failings and risks to service users in a timely manner. These actions should include:
a) Clinical leadership and oversight of workflow systems, to meet the daily and ongoing needs of service users registered at The Old Dispensary.
b) Engagement with NHS Dorset to implement resilience support and resource to the staffing team to meet the ongoing needs of service users registered at The Old Dispensary.
c) The clinical re-review and follow-up of service users with potential missed diagnosis and those impacted by the inaccurate inputting of data to confirm they had received a monitoring review and/or a medicines review, by a clinician when they had not.
d) Active participation and cooperation with external services to ensure clinical audit and oversight.
e) Evidence there is an overarching clinical governance framework and have implemented a prioritised action plan to ensure that effective and safe running of the service.
f) Clinical oversight must be introduced for all medical and non-medical clinicians to ensure they have the necessary knowledge, skills and competence to ensure delivery of safe care and treatment to service users;
i. To include long-term conditions clinical training;
ii. Training to ensure correct use and understanding of the clinical records system and clinical assessment tools, including but not limited to, accurate coding, required action following receipt of secondary care correspondence, and Asthma Control Test assessment tool.
