Condition of this registration relating to carrying out this regulated activity
The registered provider must, by 4pm on the 24 May 2022, implement an effective system to ensure that service users, at Blackpool Victoria Hospital, requiring rapid tranquilisation are assessed and monitored appropriately and are safe from harm.
a) The system must include the following areas:
i) Ensure there is an effective policy for staff to follow in relation to service users requiring rapid tranquilisation. The policy must include clear instructions for staff around the monitoring of service users after rapid tranquilisation has been administered.
ii) Ensure that service users who have received rapid tranquilisation are monitored and observations are clearly documented.
iii) Ensure there is a clear process for how medical and nursing staff will receive appropriate training and competence in the administration and monitoring of service users who require rapid tranquilisation.
iv) Ensure there is an effective system incorporating audits and monitoring data to enable staff to proactively manage immediate risks relating to rapid tranquilisation.
v) Ensure the system has appropriate clinical leads with the skills, qualifications and responsibilities to have effective oversight of rapid tranquilisation.
vi) Ensure there is effective trust board oversight of performance for example patient outcomes in relation to service users requiring rapid tranquilisation.
The registered provider must, by 4pm on the 24 May 2022, implement an effective system to identify, escalate and manage service users at Blackpool Victoria Hospital who may present with sepsis.
a) The system must include the following areas:
i) Ensure there is an effective escalation policy for service users with suspected or confirmed sepsis
that require immediate review.
ii) Ensure that sepsis six treatment is delivered to service users with suspected or confirmed sepsis, taking into account national guidance and recommendations.
iii) Ensure there is a clear process for how medical and nursing staff will receive appropriate training and competencies on sepsis identification and management. To include the use of sepsis screening tools and use of sepsis care bundles.
iv) Ensure that documentation such as the “sepsis in adults checklist” is fully completed to deliver safe care and treatment.
v) Ensure there is an effective system incorporating audits and monitoring data to enable staff to proactively manage immediate risks relating to sepsis.
vi) Ensure the system has appropriate clinical leads with the skills, qualifications and responsibilities to have effective oversight of sepsis.
vii) Ensure there is effective trust board oversight of performance for example patient outcomes, in relation to sepsis and antimicrobial prescribing and stewardship.
The registered provider must, by 4pm on the 24 May 2022, and thereafter on a fortnightly basis, the provider must submit documentary evidence, including results of board assurance reports and any audits and/or monitoring to the Care Quality Commission detailing what improvements and systems have been put in place to ensure full compliance in relation to the management of service users with sepsis and rapid tranquilisation.
