There were systems and processes in place to manage and learn from current and future performance and risks to the quality of service. However, staff had not consistently followed these processes.

At the last assessment we found staff were not reporting all incidents relating to self-harm in accordance with the provider’s own policy.

During this assessment we found the provider reviewed and updated their policy to align with their community model, which meant the threshold for reporting self-harm via the provider’s incident reporting system had increased. Staff now complied with reporting self-harm which met this threshold. However, we found that other incidents were not consistently reported. In particular, incidents where the service did not have sufficient staff on duty to provide safe care and treatment to patients were not reported consistently. There were a significant number of days where the service did not have nursing cover for the inpatient unit as well as to administer medicines, but such incidents had not been reported.
