Equity in access

There was a clear referrals process in place and most patients had access to the required healthcare services, however, staff had not always made referrals in accordance with identified needs for both their mental and physical health.

Mental Health:
At the last assessment the provider had revised their triage system to ensure patients with urgent and routine needs were reviewed and assessed promptly. This worked well for most patients; however, we identified a small number of patients where there were delays in the patient receiving their initial mental health assessment.

During this assessment, we found some patients had not been referred on to the mental health team or psychology promptly and referrals for a medication review had not been made, as documented in the patients’ records. This meant there was a delay for some patients receiving assessment and treatment, which may cause unnecessary psychological distress.

Inpatient unit:
During this assessment, we found patients on the inpatient unit did not always have their physical health and social care needs met promptly. For example, there were long delays in making a social care and physiotherapy referral for one patient. This meant patients may not have all their health and social care needs met, which may impact recovery and may cause psychological distress.
