Condition of this registration relating to carrying out this regulated activity
The Trust must ensure that Rowan 2 ward has a designated person responsible for the coordination and oversight of each shift.
The Trust must not admit any new patients onto Rowan 2 ward at Highbury Hospital, this includes any readmissions, without the prior written agreement of the Care Quality Commission. Any proposed admission must be submitted to the Care Quality Commission for consideration and approval before the patient is admitted. The Trust must provide evidence that such decisions have considered patients’ individual risks, continuity of care needs and the impact on patient safety.
The Trust must ensure the safe management of risk and prohibited items on the ward and that risks to individual patients are clearly recorded, with care plans in place to manage the identified risks.
The Trust must ensure patients receive required medical reviews that have been identified as a requirement.
The Trust must ensure staff maintain accurate and contemporaneous care records for all patients. This includes any care records relating to individual patients, for example; care plans, risk assessments and handover documents.
The Trust must ensure that staff maintain accurate and contemporaneous care records to evidence patients’ assessed nutrition and hydration needs are being met.
The Trust must ensure processes are in place to ensure staff are aware of patients’ whereabouts at all times.
The Trust must ensure staff on the ward are suitably trained and coordinated to enable an appropriate and timely response to incidents.
The Trust must provide a weekly report to the Care Quality Commission setting out the actions taken and progress made to address the risks identified on Rowan 2 ward, including management of risk and prohibited items, patient risk assessments and care plans, completion of required medical reviews, shift leadership, capacity of staff to respond to incidents and coordination and oversight of patients’ whereabouts. Each report must include evidence of actions completed, outstanding risks, responsible leads, timescales for completion and how the Trust is monitoring whether improvements are effective in practice.
