Our view of the service

We conducted an unannounced focused assessment of the service onsite from the 12 to 14 May 2025. The team comprised of 2 Inspectors, and 3 Specialist Advisors and an Operations Manager. We assessed all quality statements across safe, effective, responsive, and well-led key questions. We spoke with staff, leaders and people who used the service and stakeholder organisations.

During our inspection we identified areas of concern which required the trust to take immediate action. These were escalated to the trust’s senior leaders who acted to make improvements. Our report and judgements, including ratings, reflect the evidence we gathered during the assessment. Where possible, we have included the details of the immediate action taken by the trust following our visit.

The service did not always provide and maintain safe systems of care, in which safety was managed, monitored, and assured. Most processes and policies to plan and deliver people’s care and treatment were in line with legislation and current evidence-based good practice and standards. However, people were not always cared for in the right place.

We looked at 19 sets of patient clinical records. We spoke with 7 patients; 5 family members; and 43 members of staff.

We assessed quality statements within key questions. Each quality statement assessed is awarded a score. Details on how we score can be found on our website: https://www.cqc.org.uk/about-us/how-we-do-our-job/ratings
